中心静脉血气分析在小儿脓毒性休克时的应用价值
Value of Central Vein Blood Gas Analysis in Infantile Septic Shock
【摘要】 【目的】探讨中心静脉血气分析在小儿脓毒性休克时的应用价值。【方法】以2006年9月至2008年2月在本院ICU住院,确诊为小儿脓毒性休克且无呼吸功能障碍的患儿为观察组,根据标准分为休克代偿期组(Ⅰ组)和休克失代偿期组(Ⅱ组),经治疗休克纠正后各脏器功能基本正常的患儿作为对照组(Ⅲ组),分别测量其中心静脉血气分析、动脉血气分析并进行比较分析。【结果】Ⅲ组的动脉血与中心静脉血pH、PCO2的差异无显著性;Ⅰ组患儿在动脉血未出现酸中毒时静脉血已有明显的酸中毒表现,AVpH、VAPCO2、Pv-CO2较Ⅲ组明显升高;Ⅱ组的AVpH、VAPCO2、PvCO2较Ⅰ组有进一步升高。Ⅰ组与Ⅱ组的PvO2有逐步降低。【结论】中心静脉血气分析结合动脉血气分析能更及时地反映组织气体交换和酸碱状况,能对脓毒性休克的救治提供更及时有效地指导。
【Abstract】 [Objective] To study the value of central vein blood gas analysis in the infantile septic shock.[Methods] According the diagnostic criteria,the proven septic shock infantile inpatients in ICU of our hospital between September 2006 and February 2008 were divided into three groups:patients in compensatory stage of septic shock(Group Ⅰ),in decompensated stage of septic(Group Ⅱ) and the control group(Group Ⅲ) which consisted of patients whose function of organs was basically normal after anti-shock treatment.Central vein blood gas analysis and arterial blood gas were measured and compared in 3 groups.[Results]No significant difference of pH and PCO2 was found between the arterial blood and vein blood in the control group,while in the group Ⅰ,vein blood had acidosis manifestation compared to the arterial blood which had no acidosis manifestation.The AVpH,VA PCO2 and PvCO2 were higher in Group I and Group Ⅱ than those in Group Ⅲ,and higher in Group Ⅱ than those in Group Ⅰ,but the PvO2 decreased gradually by time in Group Ⅰ and Group Ⅱ.[Conclusion]Central vein blood gas analysis combined with arterial blood gas analysis can promptly reflect gas exchange and the acid base in the tissue,it may be an effective index for septic shock.
