类固醇激素治疗重型特发性面神经麻痹的疗效

【摘要】 目的:探讨类固醇激素对重型特发性面神经麻痹的疗效。方法:收集本院发病96h内符合特发性面神经麻痹的临床和影像诊断标准的120例患者,选取其中60例House-Breakmann评级Ⅳ、Ⅴ、Ⅵ级的重型病例分为A、B两组,每组30例,分别给大剂量甲基强的松龙与小剂量强的松治疗作对照,于1周后予H-B评级和Nottingham评分。并于1个月后记录治愈的例数,同时观察临床治愈的时间及费用。结果:1周后A组的神经功能分级构成比、Nottingham评分与B组相比差异有统计学意义,1个月后A组与B组的疗效、治愈率和物理治疗费用相比差异有统计学意义,P<0.05。结论:大剂量甲基强的松龙对重型特发性面神经麻痹的近期疗效明显优于小剂量强的松,且能明显缩短治疗的时间。

【Abstract】 Objective: To investigate the outcomes of severe acute Bell’s palsy treated with corticosteroids at various doses. Methods: A total of 120 patients who fulfilled the clinical and imaging criteria for Bell’s palsy within 96 hours from onset were evaluated. From these, 60 cases diagnosed with severe acute form of the disease based on their high grades by House-Breakmann’s system (H-B Grading IV, V and VI) were assigned to two groups to receive high-dose methylprednisolone (metypred, group A) or low-dose prednisone (group B). Clinical data, including H-B Grading and Nottingham score, were evaluated after one week. The number of patients who achieved clinical cure in groups A and B was counted at one month. Meanwhile, the time to clinical cure and the medical costs were calculated. Results: Significant differences were found between groups A and B in one-week H-B grading and Nottingham score (P<0.05), as well as in number of patients cured and medical costs for physical therapy after one month. Conclusion: High-dose metypred provides advantages over low-dose prednisone in treatment of severe acute Bell’ palsy, with better short-term outcomes and significantly fewer treatment days.
