【摘要】 目的探讨腹腔镜结直肠肿瘤手术的临床应用。方法经腹腔镜行结直肠肿瘤手术38例,其中行全结肠切除术1例,乙状结肠部分切除术6例,乙状结肠癌根治术10例,Dixon手术17例,Miles手术4例。结果34例手术成功;2例上段直肠癌因侵及膀胱和子宫、2例因术中出血而中转开腹手术。平均手术时间230 min,术中平均失血130ml,术后肠蠕动恢复平均48.2小时。2例术后出现早期排尿障碍,无术后出血、吻合口瘘、肠梗阻等并发症。随访3个月～24个月,1例腹腔内广泛转移,2例肝脏转移。结论腹腔镜结直肠肿瘤手术在技术上是可行的,具有手术创伤小、胃肠道干扰小、术后疼痛轻、术后恢复快等优点,尤其对于治疗中下段直肠癌优势明显,值得进一步探索。更多还原

【Abstract】 Objective To explore the application and clinical value of laparoscopy in colorectal neoplasms.Methods Thirty-eight patients with colorectal neoplasms were conducted with laparoscopic techniques.The procedures included pancolectomy(n=1),sigmoid partial resection of colon(n=6),sigmoid colon cancer radical resection(n=10),dixon procedures(n=17),miles procedures(n=4).Results Tirty-four cases were sucsessfully done,four cases were converted to open laparotomy for the bladder and uterus invasion of rectal cancer.Two cases with early urine retention.The average operation time was 230 min,the average blood loss was 130 ml.The average time of the recovery of intestinal function was 48.2 hours.There were no massive hemorrhage and anastomotic leakage and intestinal obstruction after operation.Conclusion Laparoscopic surgery for colorectal malignancy has the same effect with the opening operation and is well characterized for its minimal invasion,safety and quick recovery,especially in treating mid-low rectal cancer.更多还原
