【摘要】 目的:探讨胃癌组织尾侧型同源转录因子-2(CDX2)和E-钙粘蛋白(E-cad)的表达与浸润、转移及预后的关系.方法:胃癌患者86例,以正常胃黏膜15例为对照,应用SABC免疫组化方法检测CDX2和E-cad的表达.结果:正常胃黏膜上皮中E-cad完全表达,而CDX2不表达.胃癌Ⅰ~Ⅱ期CDX2与E-cad的异常表达率分别为51.2%和54.9%,Ⅲ~Ⅳ期的CDX2异常表达率下降,而E-cad的则提高,分别为25.1%和78.8%,各分期间的差异有统计学意义(P<0.05).伴有淋巴结转移的55例患者中,CDX2异常表达率为67.1%,而E-cad异常表达率为86.8%,均分别与相应的无淋巴结转移组存在统计学差异(P<0.01).此外,CDX2和E-cad的表达还与细胞分化程度、肿瘤生长方式、癌细胞浸润深度及患者5a生存率有关(P<0.05).结论:CDX2和E-cad与胃癌的发生、浸润、转移有关,可联合判断预后.更多还原

【Abstract】 AIM: To analyze the expressions of CDX2 and E-cadherin in human gastric carcinoma, and to evaluate the clinical significance of these two markers in the histologic classification, invasion, metastasis and prognosis of gastric carcinoma. METHODS: Eighty-six patients with gastric carcinoma and 15 normal controls were examined for the expressions of CDX2 and E-cadherin by SABC immunohistochemistry. RESULTS: E-cad-herin was present in all normal gastric mucomembranous epithe-lium, but CDX2 was not. The abnormal expression rates of these two markers in the stage Ⅰ-Ⅱ gastric carcinoma were 51.2% and 54.9%, respectively. In the stage Ⅲ-Ⅳ gastric carcinoma, the abnormal expression rate of CDX2 decreased (25.1%, P<0.05) but that of E-cadherin increased (78.8%, P<0.05). The CDX2 and E-cadherin abnormal expression rates (67.1% and 86.8%) of patients with lymphatic metastasis were higher than those without lymphatic metastasis. Moreover, the expression levels of CDX2 and E-cadherin were also associated with gastric carcinoma differentiation, invasion, metastasis and 5-year survival rate of patients (P<0.05). CONCLUSION: The expression of CDX2 or E-cadherin may be an important feature of gastric carcinoma. A combined detection of CDX2/E-cadherin co-expression may be beneficial in the prognosis prediction of gastric carcinoma.更多还原
