未波及关节面胫骨远端骨折的经皮内固定治疗
Minimally invasive percutaneous plate osteosynthesis technique in the treatment of distal tibia fracture
【摘要】 目的观察微创接骨板固定技术(minimally invasive plate osteosynthesis,MIPO)经皮植入三叶草钢板治疗未波及关节面的胫骨远端骨折的疗效。方法回顾本科2004年8月至2006年11月收治的19例未波及关节面的胫骨远端骨折。其中闭合性骨折15例,开放性骨折4例(均为Ⅰ度开放性骨折),伴有腓骨骨折16例。均采用间接复位、微创经皮植入三叶草钢板固定治疗。结果随访6~23个月,平均12个月,愈合时间4~7.5个月,平均5.7个月,无内固定松动、断裂及畸形,有3例因挫伤术后早期出现皮肤色泽改变,予以对症、观察后好转,其余患者术后仅出现短暂的软组织肿胀,未发现感染及皮肤坏死。结论MIPO微侵入经皮植入三叶草钢板是治疗未波及关节面胫骨远端骨折的一种理想、有效的方法。
【Abstract】 Objective To determine the results of "biologic fixation" with minimally invasive percutaneous plate osteosynthesis technique(MIPO) using anatomic fracture plate in the treatment of distal tibia fracture.Methods We retrospectively analyzed 19 cases of distal tibia fracture treated during August 2004 and December 2006,including 15 cases of close fracture,14 open fracture and 16 accompanied with fibular fracture.Results All patients obtained bone union in average 5.7 months.During average follow-up of 12 months(range 6-23 months),no plate failures or loss of fixation/reduction occurred.Conclusion These results show MIPO using anatomic fracture plate in managing distal tibia fracture can decrease soft tissue problems in fracture healing.
