【摘要】 目的比较氟脲嘧啶脱氧核苷(FUDR)或5-氟脲嘧啶(5-Fu)联合顺铂(DDP)两种诱导化疗方案治疗局部晚期鼻咽癌的近期疗效和不良反应。方法40例Ⅲ期、Ⅳa中晚期鼻咽癌患者随机分为实验组(采用DDP+FUDR方案)和对照组(采用DDP+5-Fu方案),每组20例。实验组方案为:DDP 20mg.m-2.d-1,静脉滴注,第1~5天,FUDR 500mg.m-2.d-1,静脉滴注,第1~5天;21d为1个疗程。对照组化疗方案:5-Fu代替FUDR,其余剂量、用法与实验组相同。两组连用2个疗程后休息10~14d开始放疗。鼻咽原发灶剂量DT:70~76Gy/7~8W,颈部转移灶DT:60~70Gy/6~7W。结果实验组和对照组的化疗结束时有效率分别为80%和75%(P>0.05),放疗结束时有效率100%。实验组的口腔黏膜炎严重程度明显比对照组轻(P<0.01)。结论DDP+FUDR是中晚期鼻咽癌诱导化疗的有效方案,不良反应较少。更多还原

【Abstract】 Objective To compare the therapeutic effect and adverse reaction of FUDR plus cisplatin with 5-Fu plus cisplatin followed by radiotherapy for treatment of locally advanced nasopharyngeal carcinoma.Methods 40 cases of locally advanced nasopharyngeal carcinoma were randomly divided into two groups,trial group(DDP+FUDR) and control group(DDP+5-Fu),every group included 20 patients.the trial group was DDP(20mg·m-2·d-1,1-5d,ivdrip)plus FUDR(500mg·m-2·d-1,1-5d,ivdrip),every 21 days as one course of treatment,while the control group was only FUDR changed 5-Fu,the dosage and usage were the same as the trial group.Radiotherapy followed two cycles of chemotherapy 10-14 days later.The radiotherapy was given at dose of 70-76Gy/7-8W to nasopharynx,and dose of to 60-70Gy/6-7W to neck.Results The response rates of the trial group and the control group were 80% and 75%(P>0.05) respectively,when the two cycles of chemotherapy were finished.And the response rates of both of two groups were 100% when the radiotherapy was finished.However,during the treatment,patients with severe acute oral mucositis in the trial group were less than that in the control group(P<0.01).Conclusion DDP+FUDR can be used as neo-adjuvant chemotherapy for treatment of locally advanced nasopharyngeal carcinoma,the side effect is less.更多还原
