【摘要】 目的回顾性评价支气管肺癌肿瘤滋养动脉超选择性插管化疗栓塞的技术成功率、安全性与疗效。方法在日本岛津1250MA CCD数字减影X光机的引导下,使用4F YASHIRO超滑导管及2.7F超滑微导管实施超选择性肿瘤滋养动脉插管,以超液态碘油及2~3种化疗药物(依肿瘤类型而定)对临床及(或)病理证实的原发性支气管肺癌病灶实施完全性化疗栓塞,术后随访观察临床症状的改变、肿块的影像学变化及生存率等。结果45例支气管肺癌,43例成功实施超选择性肿瘤滋养动脉插管、栓塞,技术操作成功率95.6%(43/45),无脊髓损伤发生。经过1~3次治疗后,患者临床症状明显改善97.8%(44/45);肿块明显缩小91.1%(41/45),肿块缩小6.7%(3/45),肿块无明显变化2.2%(1/45)。术后6个月、1年、2年、3年生存率分别为100%(45/45)、91.1%(41/45)、84.4%(38/45)、62.2%(28/45)。结论支气管肺癌瘤灶滋养动脉超选择性插管化疗栓塞的技术操作成功率高、安全性高,疗效明显。更多还原

【Abstract】 Objective To evaluate the safety,technical success rate,and effectiveness of transcatheter ultraselective arterial chemoembolization(TACE) for unresectable lung cancer.Methods Using Japan Shimadzu 1250Ma CCD DSA X machine,4F YASHIRO catheter and 2.7F microcatheter were employed in ultraselective chemoembolization for feeding artery of lung cancer with lipiodol and 2-3 species chemodrug.After treatment,the change of the symptoms,images of neoplasm and the survival rates were observed.Results There were 43 cases successfully complete ultraselective chemoembolization in feeding artery of lung neoplasm.Technical success rate was 95.6%(43/45) without spinal damage occuring.After 1-3 time treatment,the symptoms of the patients obviously improved in 44 cases(97.8%);neoplasm notable shrank in 41 cases(91.1%),partly shrank in 3 cases(6.7%),not shrinking in 1cases(2.2%).The survival rate at 6 months,12 months,24 months,36 months were 100%(45/45),91.1%(41/45),84.4%(38/45) and 62.2%(28/45) respectively.Conclusion The technical success rate at Ultraselective transcatheter arterial chemoembolization for feeding artery of lung neoplasm is high.the safety is high.It′s effectiveness is better.更多还原
