乙醇对糖尿病易感患者血清肿瘤坏死因子-α、白介素-1、白介素-6、谷氨酸脱羧酶抗体水平的影响

【摘要】 目的观察糖尿病易感患者酗酒时血清肿瘤坏死因子-α(TNF-α)、白介素-1(IL-1)、白介素-6(IL-6)、谷氨酸脱羧酶抗体(GADA)水平的变化。方法选择一组(n=62)有糖尿病家族史的酗酒者体重指数和血脂正常为A组(n=28)、有糖尿病家族史的酗酒者体重指数和血脂升高为B组(n=34)、2型糖尿病患者为C组(n=23)、正常对照组为D组(n=21),分别测定血清TNF-α、IL-1、IL-6、GADA水平的变化,有糖尿病家族史的酗酒者停酒3个月后再次检测血清TNF-α、IL-1、IL-6、GADA水平的变化。组间均数比较采用t检验。结果A组患者酗酒时血清TNF-α、IL-1、IL-6、GADA水平高于D组(P<0.05),B组血清TNF-α、IL-1、IL-6、GADA水平高于D组(P<0.05),C组患者血清TNF-α、IL-1、IL-6、GADA水平明显高于D组(P<0.01);糖尿病易感患者酗酒者在停酒3月后再次检测血清TNF-α、IL-1、IL-6、GADA水平下降(P<0.05)。结论糖尿病易感患者酗酒时乙醇诱发和/或加重胰岛素抵抗、胰岛β细胞的损伤,由此导致糖代谢紊乱的发生和发展。

【Abstract】 Objective To investigate the levels of TNF-α,IL-1,IL-6,GADA in consumed alcohol patients.Methods The levels of TNF-α,IL-1,IL-6,GADA in serum were determined in four groups: Group A(The normal body mass index and serum lipid in family history of diabetes,n=28),Group B(The high body mass index and serum lipid in family history of diabetes,n=34),Group C(Type-2 diabetes,n=23),Group D(Normal control,n=21).Then the levels of TNF-α,IL-1,IL-6,GADA were measured again after temperance 3 months.T-test were used to comparative each group.Results The levels of TNF-α,IL-1,IL-6,GADA in group A are higher than those of group D(P<0.05);The levels of TNF-α,IL-1,IL-6,GADA in group B are higher than those of group D(P<0.05);The levels of TNF-α,IL-1,IL-6,GADA in group C are significant higher than those of group D(P<0.01).After temperance 3 months,the TNF-α,IL-1,IL-6,GADA were decreased in high risk for diabetes consumed alcohol patients(P<0.05).Conclusion Alcohol may induce and/or enhance insulin resistance,pancreatic islet β-cell injury when the high risk for diabetes patients consumed alcohol,and then it will induce growth and development of glucose metabolic disorder.
