多层螺旋CT容积漫游技术诊断呼吸系统发育异常的价值
Diagnostic Value of Multi-slice Helical CT Volume Rendering Technique for Dysplasia of Respiratory System
【摘要】 目的:探讨多层螺旋CT容积漫游技术(Volume rendering technique,VRT)诊断呼吸系统发育异常的价值。材料和方法:搜集经影像学检查及临床诊断证实的呼吸系统发育异常22例,均行多层螺旋CT薄层扫描及支气管树VRT重建,评价其诊断呼吸系统发育异常的优势与不足。结果:VRT立体显示气管性支气管3例中,伴副心支气管、气管及支气管多发节段性狭窄1例,右上叶尖段支气管直接开口于气管2例。VRT清晰显示气管支气管狭窄14例,表现为管腔气柱变窄,解剖标志明确。VRT显示支气管肺发育不良3例,相应肺体积缩小或无充气肺组织,气管支气管不规则狭窄或不发育。直观显示支气管树呈囊、柱状扩张1例,左上隔离肺1例(手术病理证实),阻塞性肺气肿8例,纵隔疝4例。结论:MSCT-VRT可直观、立体也显示呼吸系统发育异常,但显示肺叶以下支气管狭窄,节段性肺不张、肺气肿及肺感染有一定限度。
【Abstract】 Purpose:To investigate diagnostic value of multi-slice helical CT volume rendering technique(VRT) for dysplasia of respiratory system.Materials and Methods:25 cases with dysplasia of respiratory system were collected in this study,all cases had undergone thin slice CT scanning and VRT of bronchial tree were reconstructed.The VRT advantage and disadvantage for the diagnosis of respiratory system were retrospectively evaluated.Results:There were 3 cases of tracheal bronchus,which vice ramus cardiacus bronchus,tracheal and bronchial stenosis were found in 1 case and right apical segment bronchus directly from trachea were demonstrated in 2 cases by VRT stereo-images.14 cases of tracheobronchial stenosis were clearly displayed in VRT images.VRT showed 3 cases of bronchopulmonary dysplasia with reduced pulmonary volume or no air filling pulmonary tissue and with irregular stenosis or aplasia in tracheo and bronchus.VRT directly demonstrated cystic and cylindrical dilatation of bronchus tree in 1 case and left superior pulmonary sequestration in 1 case.8 cases of obstructive emphysema and 4 cases of mediastinal herniation were clearly revealed in VRT images.Conclusions:Multi-slice CT-VRT images can clearly display dysplasia of respiratory system and make up the demonstration shortage of longitudinal range.It has a certain limit in demonstration of lobar and below lobar bronchi stenosis,local pulmonary atelectasis,infection and emphysema.
