DTI在脊髓型颈椎病中的应用价值
Value of diffusion tensor imaging in cervical spondylotic myelopathy
【摘要】 目的探讨弥散张量成像(DTI)在脊髓型颈椎病(CSM)中的应用价值。方法40例临床及影像学证实为CSM且髓内无明显T2高信号患者,行颈髓MRI和扩散张量成像检查,并分析影像表现和测量其表观弥散系数(ADC)值及各向异性分数(FA)值。结果所有研究对象的DTI均可得到颈髓的ADC图及FA图。其中约90%(36/40)患者脊髓受压部位ADC值较正常部位升高(P=0.015),约70%(28/40)患者脊髓受压部位FA值较正常部位降低(P=0.037)。结论DTI对CSM患者早期脊髓损伤具有较高的敏感性。
【Abstract】 Objective To study the value of diffusion tensor imaging (DTI) in the cervical spondylotic myelopathy. Methods Cervical MRI and DTI were performed in 40 patients with cervical spondylotic myelopathy and no T2WI abnormalities proved by clinical and radiological findings. The radiological findings were analyzed, and the apparent diffusion coefficient (ADC) values and the fractional anisotropy (FA) values were calculated. Results Both ADC and FA maps were able to be obstrained in every people. The ADC values increased at the spinal cord compression position compared with the normal spinal cord about 90% (36/40) patients (P=0.015); and the FA value decreased at the spinal cord compression position compared with the normal spinal cord about 70% (28/40) patients (P=0.037). Conclusion DTI proved to be more sensitive in the early spinal injury of cervical spondylotic myelopathy.
