尿激酶型纤溶酶原激活物、基质金属蛋白酶在肝细胞癌中的表达及其临床意义
Expression of u-plasminogen activator and matrix metalloproteinases-9 and their significance in hepatocellular carcinoma
【摘要】 目的探讨尿激酶型纤溶酶原激活物(u-PA)、基质金属蛋白酶(MMP-9)在肝细胞癌中的表达及其临床意义。方法用RT-PCR和免疫组织化学法检测45例肝细胞癌及其癌旁组织u-PA和MMP-9的mRNA及蛋白表达,7例肝血管瘤旁肝组织作为对照组。结果肝癌组织中u-PA和MMP-9mRNA阳性率均为100%(45/45);其对应的癌旁组和对照组的阳性率分别为84.44%(38/45)、28.57%(2/7)和86.67%(39/45)、57.14%(4/7);三组比较差异均有显著性(P<0.05)。免疫组化显示,u-PA和MMP-9在肝癌组中阳性率分别为75.56%(34/45)、66.67%(30/45),癌旁组分别为26.67%(12/45)、22.22%(10/45),对照组分别为28.57%(2/7)、14.29%(1/7),三组比较差异有显著性(P<0.05)。此外,在肝癌组织中u-PA、MMP-9的mRNA和蛋白的阳性率均与静脉癌栓形成、肝内外转移有关(P<0.01),与乙型肝炎、病理分级、血清AFP水平以及肿瘤直径无明显关系(P>0.05)。结论肝细胞癌中的u-PA和MMP-9mRNA及蛋白的高表达与肝癌的侵袭转移有关;检测u-PA和MMP-9有助于判断肝细胞癌的预后。
【Abstract】 [Objective] To investigate the expression of urokinase-type plasminogen activator(u-PA) and matrix metalloproteinase-9(MMP-9) and their significance in hepatocellular carcinoma.[Methods] Reverse transcription polymerase chain reaction(RT-PCR) and mmunohistochemistry were used to detect the expression of u-PA and MMP-9 in 45 cases of hepatocellular carcinoma(HCC), their adjacent liver tissues, and 7 cases of adjacent liver tissues from hemangiomas of liver (as control group). [Results] The positive rates of u-PA and MMP-9 mRNA in HCC were both 100%(45/45); the rates of their adjacent liver tissues and the control group were 84.44%(38/45), 28.57%(2/7) and 86.67%(39/45), 57.14%(4/7), respectively (P <0.05). Immunohistochemistry revealed that, among 45 cases, the positive rates of u-PA and MMP-9 were 75.56%(34/45), 66.67%(30/45) in HCC, 26.67%(12/45), 22.22%(10/45) in their adjacent liver tissues, and 28.57% (2/7), 14.29%(1/7) in the control group, respectively(P <0.05). In addition, the positive rates of u-PA, MMP-9 mRNA and protein in HCC were significantly correlated with the portal vein tumor thrombus and the presence of metastasis (P <0.01), but not correlated with the type B hepatitis, pathological stage, the level of serum alpha-fetoprotein(AFP) and the diameter of tumor (P >0.05). [Conclusions] The high level of expression of u-PA and MMP-9 in HCC might be related to the growth, invasion and metastasis of HCC, and they might be used as markers of prognosis of HCC in clinical practice.
