双水平气道正压通气对COPD患者呼吸肌肌力和运动耐力的影响
Influence of noninvasive ventilation by BiPAP on respiratory muscle strength and exercise tolerance in patients with COPD
【摘要】 目的探讨双水平气道正压通气对中、重度慢性阻塞性肺疾病(COPD)患者呼吸肌肌力及运动能力的影响。方法36例中、重度COPD稳定期患者被随机分为两组,每组18例患者。治疗组患者每天接受2h的双水平气道正压通气,共2周;对照组患者只接受氧疗。同时测量治疗前后患者呼吸肌肌力和运动耐力的变化。结果和对照组相比,机械通气治疗组呼吸肌肌力明显增强,运动耐力明显提高。结论双水平气道正压通气能增强中、重度COPD患者的呼吸肌力,提高运动耐力。
【Abstract】 [Objective] To assess the effect of bi-level positive airway pressure(BiPAP),by nasal mask,on exercise tolerance and respiratory muscle strength in patients with a clinical and spirometric diagnosis of moderate/severe COPD.[Methods] 36 patients were randomly divided into two groups.18 patients in the experimental group were treated with BiPAP via nasal mask,and the other 18 patients in the control group were treated with oxygen therapy alone.Before and after the treatment,inspiratory and expiratory muscle strength and the distance walked in six minutes were measured.[Results] Significant difference between the two groups was observed.Inspiratory and expiratory muscle strength and the distance walked in six minutes had significant difference between two groups after 2 weeks treatment(P <0.05).[Conclusion] Based on these results,we concluded that BiPAP improved respiratory muscle strength and exercise tolerance in these COPD patients.
