2型糖尿病并发周围神经病变与血清抵抗素水平的关系

【摘要】 目的探讨血清抵抗素水平与2型糖尿病(T2DM)并周围神经病变(PN)的关系。方法采用酶联免疫法测定下列2组的血清抵抗素水平。将96例 T2DM 患者分为2组,DM 并发 PN组(DM+PN,n=52),单纯 DM 组(DM,n=44),对照组(NC,n=34)。结果 DM+PN 及 DM 组血清抵抗素水平均明显高于 NC 组(P<0.05);二者相比,DM+PN 组血清抵抗素水平较 DM 组明显升高(P<0.05)。相关分析发现,血清抵抗素浓度与空腹血清胰岛素(FINS)、空腹血清内皮素(ET)、收缩压(SBP)、舒张压(DBP)及 HOMA 胰岛素抵抗指数(HOMA-IR)呈显著正相关(均 P<0.05),与HOMA-β细胞功能指数(HOMA-β)及空腹血清一氧化氮(NO)显著负相关(P<0.05)。结论 DM+PN 组血清抵抗素水平显著高于 DM 组,高抵抗素血症与 ET、NO 的相关性提示,抵抗素可能在2型糖尿病周围神经病变(PN)的发病中起一定作用。

【Abstract】 Objective To investigate the relationship between the serum resistin concentration and type 2 DM with peripheral neuropathy.Methods Ninety-six patients of type 2 diabetes pmellitus(DM) among which 52 were with peripheral neuropathy(DM + PN)and 34 healthy persons undergoing routine physical examination as controls were studied.Body weight,waist circunfference,blood pressure,body mass index(BMI),fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA-IR), and beta-cell action measured by the homeostasis model assessment(HOMA-β),and blood lipids were measured.ELISA was used to detec the srerum resistin,epithelin(ET),and nitrogen oxide(NO). Results The srum resistin level of the DM + PN patients was(26±9)ng/ml,significantly higher than that of the DM patients[(20±6)ng/ml,P<0.05].Th serum resistin level was positvely correlated with FINS,Homa-IR,systolic blood pressure,diastolic blood pressure,and ET(all P<0.05)and significantly negatively correlated with HOMA-β and NO(both P<0.05).Conclusion Resistin may play a role in the pathogenesis of type 2 DM and diabetic peripheral neuropathy.
