慢性丙型肝炎患者的血清HCV基因型和RNA含量与ALT和AST水平不相关

【摘要】 目的研究 HCV 基因型、RNA 含量与肝损伤指标的相关性。方法对来自14家医院的208例慢性丙型肝炎患者的血清进行 ALT、AST 检测,采用罗氏公司的 Cobas Amplicor HCV MonitorTest version 2.0(v2.0)试剂进行 HCV RNA 定量和 Simmons 酶切分型方法进行 HCV 基因分型检测,分析 HCV 基因型、HCV RNA 含量与 ALT、AST 水平及 AST/ALT 比值的相关性。结果本研究的208例慢性丙型肝炎患者中,基础 HCV RNA 含量与 ALT、AST 水平及 AST/ALT 比值没有相关性(r 值分别为0.093、0.092、0.009,均 P>0.05),高病毒含量组(1>8.5×10~5IU/ml)与低病毒含量组(<8.5×10~5IU/ml)比较 ALT、AST 水平及 AST/ALT 比值的差异无统计学意义(P=0.101、0.052、0.746);HCV 基因1型与非基因1型之间 ALT、AST、AST/ALT 的差异无统计学意义(P=0.406、0.461、0.528),HCV基因1型与非基因1型之间 RNA 含量差异无统计学意义(P>0.05);治疗结束时病毒应答组和非应答组比较的 ALT、AST 水平及 AST/ALT 比值的差异无统计学意义(P=0.443、0.641、0.942),随访结束时,病毒应答组和非应答组的 ALT、AST 水平及 AST/ALT 比值的差异无统计学意义(P=0.201、0.411、0.76)。结论慢性丙型肝炎患者的 HCV 基因型和 RNA 病毒含量与肝损伤指标没有相关性。

【Abstract】 Objective To investigate the relationship between HCV genotype,serum viral load and the parameters,including ALT and AST,reflecting the damage of hepatic function in chronic hepatitis C patients.Methods The HCV RNA levels were determined with Cobas Amplicor Monitor Test,version 2.0, and HCV genotypes were examined by the means of PCR products of 5′ NTR digested with restriction endonucleases,and ALT and AST levels were detected by automatic biochemistry analyzer in the serum of 208 chronic hepatitis C patients.The relationship of HCV genotype to HCV RNA loads,ALT,AST levels and AST/ALT ratio were analyzed with the statistic software.Results There were 208 chronic hepatitis C patients in this study.The ALT,AST level and AST/ALT ratio were not related with the HCV RNA load, with coefficient correlation 0.093,0.092,0.009 respectively(P>0.05),the difference of ALT,AST level and AST/ALT ratio between high viral load (serum HCV RNA≥8.5×10~5 IU/ml) group and low viral load (serum HCV RNA <8.5×10~5 IU/ml) group was not significance (P=0.101,0.052,0.746 respectively).No significant difference of ALT,AST level and AST/ALT ratio was found between patients with genotype 1 and non 1 HCV infection,and the HCV RNA content was not difference between HCV genotype 1 and non 1 group also.In the analysis of ALT,AST and AST/ALT ratio on the effect of IFN treatment,there was no difference between sustained viral response and nonresponse group (P=0.201, 0.411,0.761 respectively).Conclusion There is no relationship between HCV genotypes,HCV RNA load and the parameters reflecting the damage of hepatic function.
