【摘要】 目的探讨甲状腺功能亢进症(甲亢)患者血抵抗素水平与胰岛素抵抗(IR)的关系。方法分别测定32例甲亢患者及40例健康对照(NC)者空腹血清抵抗素、胰岛素、血脂、血糖等水平,测量身高、体重,计算BMI、IR指数,分析相关因素。结果与NC组比较,甲亢患者空腹血清抵抗素、胰岛素、FPG及HOMA-IR明显升高(11±6μg/Lvs7±4μg/L,P<0.01;7.76±2.92mU/Lvs6.37±2.67mU/L,P<0.05;5.38±0.92mmol/Lvs4.79±0.48mmol/L,P<0.01;2.20±1.03vs1.43±1.12,P<0.01);甲亢时血抵抗素与游离三碘甲腺氨酸、游离甲状腺素及HOMA-IR正相关(r=0.448,0.405,0.393,P=0.011,0.023,0.036)、与高敏促甲状腺激素负相关(r=-0.361,P=0.045),校正年龄和BMI后偏相关分析显示,血抵抗素水平仍与HOMA-IR呈正相关。结论抵抗素可能是甲状腺功能亢进时发生IR和糖代谢紊乱的重要因素之一。更多还原

【Abstract】 Objective To investigate the relationship of serum resistin concentration with insulin resistance in patients with hyperthyroidism. Methods The fasting serum resistin concentration was measured with enzyme immunoassay in 32 patients with hyperthyroidism and 40 healthy controls,and fasting plasma glucose and insulin,body height,body weight were measured to calculate body mass index(BMI) and insulin resistance (HOMA-IR).Correlated factors of serum resistin were analyzed. Results Hyperthyroidism group versus control group showed higher serum levels of resistin [(11±6)vs(7.09±4.36)μg/L],FIns[(7.76±2.92)vs(6.37±2.67)mU/L],FPG[(5.38±0.95)vs(4.79±0.48)mmol/L],and HOMA-IR[(2.2±1.0)vs(1.4±1.1)],and lower level of HDL-C [(1.0±0.4)vs(1.2±0.3)mmol/L],(all P<0.05-0.01).In patients with hyperthyroidism,resistin level showed a significant correlation with FT3(r=0.448,P=0.011),FT4(r=0.405,P=0.023), HOMA-IR(r=0.392,P=0.036)and sTSH(r=-0.361,P=0.045).After adjustment for age and BMI,a partial correlation analysis showed that serum resistin concentration in patients with hyperthyroidism was correlated with HOMA-IR. Conclusion Resistin might play important roles in both glyco-metabolic disorder and insulin resistance in patients with hyperthyroidism.更多还原
