胰岛素抵抗和胰岛β细胞分泌功能对妊娠糖尿病的影响

【摘要】 目的探讨胰岛素抵抗(IR)和β细胞分泌功能对妊娠糖尿病(GDM)的影响。方法观察妊娠中晚期孕妇糖耐量正常(NGT)者35例、糖耐量减低(IGT)者33例和GDM者38例在不同糖耐量状态下的IR和β细胞分泌功能,采用HOMA-IR和HOMA-β分别评价IR和β细胞分泌功能。结果IGT组的HOMA-IR明显高于NGT组(P<0·05),两组的HOMA-β的差异无统计学意义;GDM组的HOMA-IR明显高于NGT和IGT组(P<0·05),HOMA-β明显低于NGT和IGT组,差异均有统计学意义。结论妊娠中晚期孕妇IGT阶段胰岛素早期分泌功能受损,到GDM阶段兼有严重的IR和β细胞缺陷。从NGT、IGT到GDM,随着糖代谢紊乱的不断进展,IR逐渐加重,胰岛β细胞功能进行性减退。

【Abstract】 Objective To investigate the changes of insulin resistance (IR) and function of islet β cell in the process of GDM. Methods 35 subjects with NGT, 33 IGT and 38 GDM were observed. HOMA-IR was applied to assess the status of insulin resistance, and HOMA-β was used to evaluate the basic function of islet β cells respectively. Results HOMA-IR was higher in subjects with IGT (1.17±0.05) than that with NGT (0.88±0.04) (P<0.05),and it was higher in subjects with GDM (1.56±0.06) than that with NGT and IGT (P<0.05). At the same time,HOMA-β was lower in subjects with GDM (2.98±0.13) than that with NGT (5.12±0.04) and IGT(5.10±0.05)(all P<0.05). Conclusions During the middle and advanced stage of pregnancy, the subjects with IGT show a defect of the early phase of insulin secretion,and the patients with GDM have more severe IR and defect of β cell function.With the deterioration of glucose metabolism from NGT to GDM,the function of islet β cells is progressively decreased.
