【摘要】 目的探讨干扰素联合宫颈环形电切术(LEEP)在治疗宫颈上皮内瘤变(CIN)中的作用。方法对2003年1月至2005年1月大连市妇幼保健院收治,经细胞学、阴道镜定位活检、宫颈管搔刮术病理检查明确诊断为CINⅡ～Ⅲ级的60例病人,进行前瞻性随机对照研究。并对60例病人行LEEP前应用第2代杂交捕获试验(HC2)检测高危型人乳头瘤病毒(HPV)DNA,其中30例病人术前、术后加用干扰素治疗3个疗程为研究组,同时将单纯行LEEP的30例病人作为对照组。分别于治疗后3、6、9、12个月行液基细胞学检查(TCT)及阴道镜下可疑病灶活检,6、12个月行HC2检测以判断两组的疗效。结果60例病人随诊中均无CIN残留及复发。治疗前两组各有2例HPV阴性。研究组术后6个月复查TCT3例异常,HC2检测HPV27例阴性,阴性率90%;术后12个月治愈率100%。对照组术后6个月复查TCT16例异常,HC2检测HPV13例阴性,阴性率43.3%;术后12个月复查TCT4例异常,HC2检测HPV19例阴性,治愈率63.3%。术后6个月及12个月研究组与对照组治愈率比较,研究组优于对照组(P<0.05)。结论干扰素联合LEEP治疗CIN有双重阻断作用,提高了CIN病人一次性治愈率。更多还原

【Abstract】 Objective To study the effect of combining interferon(INF)and LEEP to treat CIN.Methods Prospective,random and control study was progressed in sixty patients with CINⅡ-CINⅢ.Before carrying out LEEP to these patients,all women were performed high-risk HPV DNA detection by the method of HC2.Among these patients,thirty women were assigned to be the studying group in which the patients were added to use INF for three courses of treatment before and after LEEP.The other thirty women who carried out LEEP simply were assigned to be the control group.All patients were examined by liquid-based ThinPrep cytology test(TCT)and colposcopic site-specific biopsies to doubtful focus of infection in the third,sixth、ninth and twelfth month after treating.And to judge the curative effect of two groups by using HC2 in the sixth and twelfth month after treating.Results Sixty patients were not detected residue and recidivation of CIN diagnosed by colposcopy and site-specific biopsies to doubtful focus of infection.In the studying group,3 women were abnormal detected by TCT,27 women were negative detected by HC2,the negative rate was 90%,when LEEP ended six months;the cure rate was 100% when LEEP ended twelve months.In the control group,16 women were abnormal detected by TCT,13 women were negative detected by HC2,the negative rate was 43.3% when LEEP ended six months;4 women were abnormal detected by TCT,19 women were negative detected by HC2,the cure rate was 63.3% when LEEP ended twelve months.In the sixth and twelfth month after LEEP,the difference was distinguished when we compared the cure rate between the studying group and the control group.The studying group surpassed the control group.Conclusion There is double interrupted effect by combining INF and LEEP to treat CIN.It can raise the one cure rate of the patients with CIN.更多还原
