小肝癌无水乙醇灭活后超声引导瘤区注射超抗原的局部免疫应答

【摘要】 目的研究小肝癌经皮无水乙醇治疗(PEIT)后瘤区连续注射超抗原局部的免疫应答。方法选取34例小肝癌患者,均于无水乙醇注射治疗原位灭活后第3、第4和第7周瘤区经皮局部注射超抗原生物制剂高聚生(PHIT),每次注射1600U。比较注射高聚生(HAS)前后治疗区CD3、CD4、CD8、CD57和CD68浸润变化情况。结果CD3、CD4、CD57和CD68局部浸润较注射高聚生前有明显增高(P<0.01),且持续至注射后第5周仍高于治疗前(P<0.05)。结论超声引导局部注射高聚生能够有效地增强肝癌患者局部抗肿瘤细胞免疫,可能具有一定的抗肿瘤复发和降低肝癌复发率的作用。

【Abstract】 Objective To study the local immune response by injecting highly agglutinative staphylococci(HAS)as superantigen into the tumor areas in liver cancer patients received percutaneous anhydrous ethanol injection therapy(PEIT)under ultrasound guidance.Methods All the 34 cases with small liver cancer proved by pathology were treated with PEIT.Then the patients were given injection of HAS 1,600 units into the local tumor areas at the 3rd,4th and 7th weeks.The patients underwent percutaneous biopsy under ultrasound guidance for inactivating the focal hepatic tumors before percutaneous HAS injection therapy(PHIT).One week later,all the samples were stained with HE and immunohistochemicals.The infiltration of CD3,CD4,CD8,CD57and CD68 in local treatment areas were compared before and after PHIT.Results(1)After local injection of HAS,the degrees of infiltration of CD3,CD4,CD57 and CD68 were increased in different extents compared with those of pre-PHIT(P<0.01),and lasted to one month after HAS injection(P<0.05).The increased immunocytes were functioning powerfully after PHIT.(2)After local HAS injection,the local positive infiltration of IL-2 was increased significantly than that of pre-PHIT(P<0.05).Conclusions The local cellular immunity of liver carcinoma may be more improved after the tumor was treated by PHIT.It might repel tumor recrudescence and reduce the recrudescence rate.
