肺实变的超声特征及其诊断价值
Ultrasonographic Characteristics of Pulmonary Consolidation and Its Significance
【摘要】 目的探讨肺实变的超声特征及其诊断价值。方法对125例实变肺的二维及彩色多普勒超声表现进行回顾性对比分析。结果含气或含液管状结构以及分支状血流信号是超声诊断肺实变的重要特征。彩色多普勒超声显示实变肺内血流信号有较高敏感性(92.7%),对含气或含液管状结构和肿块的检出率也高于灰阶超声,差异有显著性(P<0.05)。超声在确定肺实变方面优于X-线检查(P<0.005),与CT相比差异无显著性(P>0.05)。结论彩色多普勒超声在肺实变的诊断中优于灰阶超声,是对X-线、CT等其他影像方法的一个很好补充。
【Abstract】 Objective To explore the ultrasonographic characteristics and application value in pulmonary consolidation.Methods Two-dimensional(2-DE) and color Doppler ultrasonographic(CDUS) findings were analyzed in 125 consolidated lungs proved by clinical and the results were compared with X-ray and CT.Results The main ultrasonographic characteristics were tubular structures filled with air,or liquid and dendritic blood flow signals in pulmonary consolidation.The sensitivity of CDUS indicating flow signals was 92.7%(51/55).The sensitivity for diagnosing pulmonary consolidation was:90.0%(63/70)in 2-DE,100%(55/55)in CDUS,75.2%(94/125)in X-ray and 96.8%(121/125)in CT.There were significant differences in detecting the tubular structures and tumors of the consolidated lungs between CDUS and 2-DE(P<0.05).US was superior to X-ray in demonstrating pulmonary consolidation (P<0.005).However,there were no significant differences between US and CT (P>0.05).Conclusions CDUS is superior to 2-DE in diagnosing pulmonary consolidation.To X-ray and CT,CDUS is the best supplemental modality for other kinds of imaging.
