【摘要】 目的:观察阴道放置米索前列醇对无痛人工流产术全麻用药量的影响。方法:选择ASAⅠ~Ⅱ级,无呼吸系统、心血管系统疾病,无胃肠功能紊乱及非变态反应体质的患者60例。依随机数字法分为观察组(n=30)和对照组(n=30)。观察组患者术前3 h阴道放置米索前列醇400μg。两组患者摆好体位,在手术医师开始消毒时静脉注射芬太尼0.8μg.kg-1,匀速注射丙泊酚1.5~2.0 mg.kg-1,患者入睡后开始手术。术中根据病人情况酌情追加丙泊酚0.5~1 mg.kg-1。观察患者丙泊酚用量、体动反应及宫颈软化情况。结果:①丙泊酚用量对照组明显高于观察组(P<0.05)。②术中发生体动对照组(8例,26.7%)明显高于观察组(2例,6.7%,P<0.05)。③宫颈软化不良对照组20.0%,而观察组3.3%,对照组明显高于观察组(P<0.05)。结论:在无痛人工流产术前3 h阴道放置米索前列醇,能有效地软化宫颈,减轻扩宫时疼痛刺激,减少全麻药用量,值得临床推广应用。更多还原

【Abstract】 Objective:To observe the effect of Misoprostol on the dose of general anesthetic medication for painless induced abortion.Methods:60 ASAⅠ～Ⅱpatients scheduled for painless induced abortion without cardiopulmonary diseases and gastrointestinal dysfunction and hypersensitivity were randomly divided into observe group(n=30) and control group(n=30).The patients in observe group were deposited with 400 μg Misoprostol into vagina at 3h before operation.Before the operation,patients in tow groups were injected intravenously with 0.8 μg·kg-1 fentanyl and 1.5~2.0 mg·kg-1 propofol.The operation began as soon as the patients fall asleep.According to the reaction of patients supplemental propofol was 0.5~1.0 mg·kg-1.The dose of propofol,body movement and malacosis of cervix were observed.Results:①The dose of propofol in control group was higher than that in observe group(P<0.05).② the ratio of body movement in control group(8 cases,26.7%) was higher than that in observe group(2 cases,6.7%,P<0.05).③ The ratio of ill cervix malacosis in control group(20%) was higher than that in observe group(3.3%,P<0.05).Conclusion: Misoprostol deposited into vagina at 3h before painless induced abortion can effectively emolliate cervix,alleviate pain and reduce the dose of general anesthetic medication,that is good for clinical application.更多还原
