【摘要】 目的:探讨短暂肝实质灌注异常(transient hepatic perfusion disorders,THPD)的多层螺旋CT(multislice computed to-mography,MSCT)表现及其产生病理基础。方法:2005年1月~2006年11月间因肝病作MSCT的318例检查者中发现31例THPD。全部病例常规上腹部平扫;注射对比剂后20~25s行肝动脉期扫描,60~70s行门静脉期扫描;18例行5~10min延迟期扫描。对31例THPD影像资料进行回顾性分析。结果:31例THPD中,肝肿瘤病变18例,肝非肿瘤病变12例,壶腹周围癌1例,均经临床、手术或/和病理证实。将THPD分为四型:①肝叶、段型;②楔型;③不规则型;④弥漫型。结论:THPD的MSCT表现多具特征性,正确识别THPD,对提高肝脏病变诊断正确性、避免过高估计肝脏病变具有重要意义。

【Abstract】 Objective:To study the multislice computed tomography (MSCT) features of transient hepatic perfusion disorders(THPD), and to discuss its pathologic basis. Methods:In 11 months, MSCT studies of 318 patients with liver diseases were performed. Unenhanced MSCT images were routinely obtained before injection of contrast material, then MSCT scans were performed during the hepatic arterial phase (HAP) (20~25seconds) and the portal venous phase (PVP) (60~70seconds), respectively, and delayed phase scans of the local lesions in 18 patients were also acquired 5~10 minutes after the initiation of injection. 31patients were identified to have THPD. The findings on MSCT of the 31 patients with THPD were analyzed retrospectively. Results:Of the 31 patients with THPD, 18 patients have the hepatic tumors, 12 patients have the other hepatic diseases, 1 patient has the pei-ampullary carcinoma. All the cases were proved by clinical or pathology. THPD was divided into four types, i. e. liver segmental type, wedge-shaped type, irregular type, diffuse type.Conclusion:THPD had characteristic MSCT features. It is highly important distinguish THPD, which could increase the level of diagnosing diseases inside liver and avoid to over estimate diseases inside liver.
