【摘要】 目的观察采用调整Parks法手术量治疗有代偿头位(compensatory head posture,CHP)的先天性眼球震颤(congenital nystagmus,CN)的临床效果。方法对28例伴有CHP的CN患者,采用调整Parks方法的手术量行中间带移位术,当代偿头位为15°~25°、25°~30°、30°~40°、>40°时,从慢相侧到快相侧4条水平直肌手术量分别为7050mm、6446mm、7557mm、7658mm。对比观察术前、术后视力及CHP情况。结果与术前相比,术后单眼或双眼同时注视时的最佳矫正视力和第一眼位视力均有显著提高。CHP基本消除者(CHP<10°)21例,明显改善者(CHP>10°)6例,无变化者1例,与术前相比疗效显著。3例伴水平斜视者全部矫正为正位。结论调整Parks法手术量适用于治疗各种大小CHP的CN患者。更多还原

【Abstract】 Objective To evaluate the clinical effects of treating congenital nytagmus(CN)with compensatory head posture(CHP)by modified Parks method.Methods Twenty-eight patients with CN were selected from the clinic,modified Parks shift of neutral zone was conducted on patients with nystagmus.When the compensatory head posture was from 15° to 25°,from 25° to 30°,from 30° to 40°,more than 40°,the four horizontal rectus muscles were retreated for 7 050 mm,6 446 mm,7 557 mm,7 658 mm from the slow side to the nystagmus side.Results After surgery,both single and binocular best-corrected vision was improved in almost all cases.Compensatory head posture basically disappeared(CHP<10°)in 21 patients,6 patients were obviously relieved,while 1 patient had no change.There was a marked correction.In 3 patients with complicating strabismus before operation,all were corrected.Conclusion Modified Parks shift of neutral zone can effectively suit for the CN with CHP.更多还原
