椎体成形术中国产骨水泥配制比例的实验研究

【摘要】 目的:研究国产骨水泥最佳的配制比例。材料与方法:(1)用国产骨水泥和造影剂,参照国外骨水泥配制比例(对照组),选取其中一较为理想的配制比例配制水泥(试验组),两组各制成水泥柱10个,进行耐压和应力的生物力学测试,比较其差异;(2)取5具尸体相邻胸腰段椎体20个,分成两组,一组作为对照,另一组注射骨水泥,然后分别进行耐压测定,比较有无差异;(3)取健康家犬12只,用两种比例骨水泥进行经皮腰椎椎体成形术,观察所调制的骨水泥注射难易程度、椎体成形情况和并发症发生情况。结果:国产骨水泥的最佳配制比例为粉剂、单体和造影剂的比例为4:2.6:1(g、ml、ml),术中用该种比例配制骨水泥,操作方便;两种水泥块在生物力学上无显著性差异;在尸体标本上注射该种骨水泥可明显提高椎体强度和韧性,有显著性效果。结论:粉剂、单体和造影剂的比例为4:2.6:1(g、ml、ml)调制成的骨水泥可以更好地使临床应用经皮椎体成形术来治疗椎体疾病。

【Abstract】 Purpose:To study the best ratio of bone cement mixed for applying percutaneous vertebroplasty in clinical practices.Materials and Methods:(1)Bone cement were disposed in different powder-to-monomer ratios with domestic PMMA and the concretionary phases of cement were observed.Then the most ideal ratio of the mixed cement was picked out and ten cement columns were made with it as a trial roup.The other ten cement columns recommended by the literatures were severed as contrast one.These specimens were measured about the biological mechanics in loading and the data of the results were compared.(2) Twenty adjacent thoracic and lumbar vertebral bodies were removed intact from five human corpses were divided into two groups,one was injected by PMMA,the other was severed as contrast.Then twenty vertebral bodies were measured the biological mechanics and the treatment role was evaluated.(3) Twelve healthy dogs were used to trial on PV in lumbar to observe the difficulty extent of injecting this bone cement,and CT was used to evaluate both the diffusions of PMMA and the complications.Results:The most ideal powder-to-monomer ratio is 4:2.6:1(g、ml、ml) respectively.It is convenient and adequate time to inject this type cement.Two types cement column were no great differences in loading measures(P>0.05).By injecting this kind cement,the loading of these vertebral bodies were increased remarkably(P<0.01).The results of bone cement dispersion in the dog lumbar bodies were significant differences with the two ratios mixed cement (P<0.05).Conclusion:The ideal bone cement powder-to-monomer ratio is 4:2.6:1 Powder(g)、monomer(ml)、contrast(ml).
