相关信号转导分子在创伤性兔膝骨关节炎发病中的变化
Relevant signal transduction molecule in the pathogenesis of posttraumatic rabbit knee osteoarthritis
【摘要】 目的:在创伤性兔膝骨关节炎的发病机制中关节软骨细胞凋亡、诱导型一氧化氮合酶、基质金属蛋白酶组织抑制因子3、尿激酶型纤溶酶原激活物及其受体蛋白均会发生一些变化,观察其相关特征,从而确立骨关节炎"细胞通讯"的病理机制。方法:实验于2004-07/2006-07在兰州大学第一附属医院中心实验室及甘肃中医学院实验室完成。①实验动物:将36只健康青紫蓝家兔(雌雄各半)随机分成正常组、模型组各18只。②实验方法:模型组用硫喷妥钠30mg/kg腹腔麻醉,在无菌条件下取双侧膝关节内侧切口长约3cm,直视下探查关节腔无原发病变后,用眼科剪伸入切断前后交叉韧带、内侧副韧带、完整切除内侧半月板进行造模,正常组不作任何处理。③实验评估:运用流式细胞分析法、免疫组化法分别于6、8周两个时期进行DNA倍体含量、基质金属蛋白酶组织抑制因子3、尿激酶型纤溶酶原激活物及其受体蛋白、诱导型一氧化氮合酶蛋白阳性强度观察。结果:参加实验36只家兔,进入结果分析20只。①诱导型一氧化氮合酶蛋白表达:6周时各组间差异无显著性,8周时模型组损伤胶原化区与各组间差异显著(P<0.05)。②基质金属蛋白酶组织抑制因子3蛋白表达:6周时各组间比较差异有显著性,正常组6周和8周比较差异有显著性(P<0.05)。③细胞凋亡情况:8周时正常组与模型组DNA比率差异有显著性、但细胞凋亡均数差异无显著性。④尿激酶型纤溶酶原激活物及其受体蛋白表达:6,8周时各组间比较差异有显著性;正常组与模型组损伤胶原化区、正常组与模型组增殖肥厚区、正常组与模型组无损伤区;损伤胶原化区与增殖肥厚区、损伤胶原化区与无损伤区间差异有统计学意义(P<0.05);正常组和模型组6周和8周比较差异均有显著性(P<0.05)。结论:骨关节炎病理机制中存在破坏与修复相互拮抗平衡的分子"多轴心机制"。
【Abstract】 AIM: Cartilage cell apoptosis, inducible nitric oxide synthase (iNOS), tissue inhibitor of metalloproteinase (TIMP)-3, urokinase-type plasminogen activator (UPA) and its receptor protein in the pathogenesis of rabbit osteoarthritis may change. This article is aimed to design their characteristics in order to define the pathomechanism of intercellular communication of osteoarthritis. METHODS:The experiment was performed at the Central Laboratory of First Affiliated Hospital of Lanzhou University and Laboratory of College of Traditional Chinese Medicine from July 2004 to July 2006. ①Totally 36 healthy chinchilla rabbits(female and male be in half)were divided into normal group and model group randomly with 18 in each group. ②Rabbits in the model group received thiopental sodium 30 mg/kg intraperitoneally. The inner knee joints of two sides were fetched in no germ condition about 3 cm in length. If there was no general disease, an eye scissors was used to cut off the anterior, posterior cruciate ligament, medial collateral ligament, and full medial meniscus was ectomized to form a model. Rabbits in the normal group did not receive any intervention. ③DNA content and TIMP-3, UPA and its receptor protein, iNOS positive strength were determined at weeks 6 and 8 by flow cytometry method and immunohistochemical method, respectively. RESULTS: Totally 20 rabbits of the 36 rabbits were involved in the result analysis. ①No significant differences were found in iNOS level among each group at the sixth week, but there were significant differences between the injured collagen of rabbits of the model group and other groups at the eighth week (P < 0.05). ②Significant differences were found in TIMP-3 level among each group at the sixth week, and significant differences were also found between that at week 6 and week 8 in the normal group (P < 0.05). ③Significant differences were found in DNA ratio between the normal group and model group at week 8, but no significant differences were found in apoptosis mean. ④There were significant differences in UPA level and its receptor protein among each groups at weeks 6 and 8. There were significant differences between normal group and collagen injuring group, normal group and hyperplaria hypertrohy group, normal group and no injured group, collagen injuring group and hyperplaria hypertrohy group, collagen injuring group and no injured group (P < 0.05). At weeks 6 and 8, significant differences were found between normal group and model group (P < 0.05). CONCLUSION: Molecular multi-axes system exists in the pathogenesis of osteoarthritis. Moreover, destruction is compatible with recovery in this system.
