癫儿童脑内代谢变化的~1HMRS研究
Research of magnetic resonance spectroscopy on changes of the intracerebral metabolism in epileptic children
【摘要】 目的:探讨磁共振质子波谱(1HMRS)在儿童癫中的应用。方法:对24例颞叶癫及额叶癫患儿进行1HMRS采集。测定N-乙酰天门冬氨酸(NAA)、肌酸(Cr)、胆碱复合物(Cho)、乳酸(Lac)、谷氨酸及谷氨酰胺(Glu及Gln)、r-氨基丁酸(GABA)等代谢产物,并以15例非颅内病变患儿作为对照。结果:癫组病侧与癫组对侧和对照组NAA/(Cho+Cr)比较,差异有显著性(P<0.01),癫组病侧与癫组对侧和对照组NAA/Cho比较,差异亦有显著性(P<0.01)。结论:1HMRS可无创探测小儿癫癎致癫灶内神经元受损及功能异常的代谢生化改变,有助于小儿癫的定位诊断。
【Abstract】 Objective:To explore the application of magnetic resonance spectroscopy (1HMRS) in the diagnosis of pediatric epilepsy. Methods: 1HMRS were performed in 24 children with temporal lobe epilepsy (TLE) and frontal lobe epilepsy, 15 children without intracranial disease were used as controls. The concentration of N-acetylaspartate(NAA), Creatine (Cr), Choline-containing (Cho), glutamate and glutamime (Glu/Gln) and lactate (Lac) were measured in all epilepsy and controls. Results: The concentration of NAA and ratio of NAA/(cho/cr) in epileptogenic foci were significantly lower than that of sites on the opposite and control group (P<0.01). Conclusion: 1HMRS can detect the neuron injury and changes of cerebral metabolism in pediatric epilepsy.
