妊高征患者相关细胞黏附分子的表达研究
Study on the Expression of Cell Adhesion Molecules in Patients of Pregnancy Induced Hypertension
【摘要】 目的:探讨细胞黏附分子(CAMs)在妊娠高血压综合征(PIH,妊高征)发病中的作用及与妊高征病情严重程度之间的关系。方法:采用流式细胞术(FCM)和酶联免疫吸附法(ELISA)检测100例妊高征患者(轻度31例,中度36例,重度33例)和30例正常晚期孕妇(对照组)的可溶性血管细胞黏附分子-1(sVCAM-1)、细胞间黏附分子-1(sICAM-1)以及白细胞β2整合素(CD11b)、血小板α颗粒膜糖蛋白(CD62p)的表达水平,并进行各组统计学分析。结果:妊高征患者的sVCAM-1、sICAM-1、CD11b、CD62p表达水平均显著高于正常对照组,差异有显著性(P<0.01及P<0.05);轻度PIH患者sICAM-1水平与对照组比较,差异无显著性(P>0.05),而其sVCAM-1、CD11b、CD62p水平与对照组比较差异有显著性(P<0.05),中度PIH患者sVCAM-1、sICAM-1、CD11b、CD62p表达水平与轻度PIH患者比较,差异有显著性(P<0.01及P<0.05),重度PIH患者sVCAM-1、sICAM-1、CD11b、CD62p水平与中度PIH患者比较,差异有显著性(P<0.01及P<0.05),相关分析显示,CD11b、CD62p与sVCAM-1及sICAM-1显著正相关(r分别为0.567、0.598、0.503、0.487,P均<0.05)。结论:血管内皮细胞损伤是妊高征的重要发病机理,血小板活化、细胞黏附分子的表达水平升高与妊高征的发生发展有关,表达水平升高越显著妊高征患者病情就越重,检测其水平对妊高征的诊断、观察病情及防止子痫的发生均有意义。
【Abstract】 Objective To discuss the effect of the cell adhesion molecules (CAMs) in the patients of pregnancy induced hypertension and the relationship between them. Methods 100 cases of PIH (31 cases of mild PIH patients, 36 cases of moderate PIH patients, 33 cases of severe PIH patients) and 30 cases of normal late pregnancy women were determined with FCM and ELISA. Each subject was determined the level of sVCAM-1、 sICAM-1、CD11b and C D62p before giving the birth. Results The levels of sVCAM-1、sICAM-1、CD11b and CD 62p in group of pregnancy induced hypertension were significantly higher than that of normal controls (P < 0.01 or P < 0.05); the levels of sICAM-1 had no difference between mild PIH patients and normal late pregnancy women (P > 0.05), but the levels of sVCAM-1、CD11b and CD 62p were higher than that of normal controls (P < 0.01 or P < 0.05), the levels of sVCAM-1、sICAM-1、CD11b and CD62p in the plasma of moderate PIH patients were higher than that in mild PIH patients (P < 0.01 or P < 0.05), the levels of sVCAM-1、sICAM-1、CD11b and CD 62p in the severe PIH patients were higher than that in moderate PIH patients (P < 0.01 or P < 0.05). Correlation were found between CD11b and sICAM-1, CD11b and sICAM-1, CD62p and sVCAM-1, CD62p and sICAM-1. Conclusion Endothelium cell injury plays a major role in pathogenesis of PIH. Platelet activation、CAMs may involve in the pathogenesis of PIH , the levels of the CAMs were more and more high in the PIH patients. The research confirmed that the detection of the markers was useful for diagnosis and monitoring the clinical course to PIH, avoiding edampsia occurrence.
