【摘要】 目的:比较不同药物配伍方案在介入治疗早期股骨头缺血性坏死中的疗效差别。方法:将120例患早期股骨头缺血性坏死的患者随机分为两组,每组60例,第一组使用复方丹参15 ml、654-2 20 mg、尿激酶10万U和低分子右旋糖酐150 ml血管内介入治疗;第二组应用罂粟碱30 mg、尿激酶40万U、654-2 20 mg和复方丹参30 ml。注药前后分别作血管计数,以观察患侧股骨头的血供改变情况。术后24 h制动,72 h可拄双拐下地活动,并要求患者术后拄双拐免负重6个～12个月,术后常规口服肠溶阿司匹林、钙片和鱼肝油半年以上,以加强疗效,1 a内,每隔三个月用复方丹参液20 ml配低分子右旋糖酐500 m1,静脉液注,1次/d,连续15 d。一月后复治一次,0.5 a后再复治一次。1 a后复查MRI并与治疗前比较。结果:两组差别无统计学意义。(P﹥0.05)结论:血管内介入治疗对股骨头血液供应起到了改善作用,是一种创伤小、安全而有效的方法,术后制动免负重以及术后常规、规律用药是巩固疗效的关键。更多还原

【Abstract】 Objective To compare the curative effect in different drugs for early Avaseeular necrosis of the femoral head(ANFH).Methods 120 patients with ANFH were randomly divided into two groups of 60 cases.The first group were therapied by Fufang Danshen 15ml,654-2 220 mg,urokinase 100,000 units and D-40 150 ml through vascular interventional;The second group were therapied by papaverine 30 mg,urokinase 400,000 units,654-220 mg,Fufang Danshen 30 ml.To observe the change of the blood supply in femoral head by counting vascular before and after each injection respectively.Postoperative 24 hours Braked,after 72 hours patients activities use crutches,and the patients were asked not weight loading in 6 to 12 months.Conventional oral enteric-coated aspirin,calcium tablets and cod liver oil over six months to enhance the efficacy.within a year,Fufang Danshen 20 ml and D40 500 ml injected through intravenous,once a day,for 15 consecutive days in every three months. Retreatment after one and six months . To compare the curative effect by MRI after one year. Results There was no significant difference between the two groups ( P< 0.05). Conclusion vascular interventional treat- ment of the femoral head is a small trauma, safe and effective method which can improve the blood supply.Braking,No weight loading,conventional and regular Medication have played a key role in after treatment.更多还原
