【摘要】 目的探讨连续性血液滤过(continuous hemofiltration)治疗顽固性心力衰竭合并肾功能不全的疗效及安全性。方法选择2001年1月至2006年6月在襄樊市第一人民医院肾内科住院的35例顽固性心力衰竭合并肾功能不全患者,25例进行连续性血液滤过治疗(治疗组),10例因经费原因行血液透析治疗(对照组),比较两组治疗前后临床症状的改善,血肌酐(Scr)、尿素氮(BUN)、左室射血分数(LVEF)、心排血量的改变以及并发症的情况。结果治疗组临床症状均明显改善,治疗后患者 LVEF 及心排血量(CO)均明显增加(P<0.01),血液滤过并发症少;而对照组患者临床症状改善不明显,LVEF 及 CO 无明显增加,透析并发症多。结论连续性血液滤过能清除水及毒素,并保持血流动力学稳定,是一种安全、有效的治疗顽固性心力衰竭合并肾功能不全的方法。更多还原

【Abstract】 Objective To investigate the effects and safety of the treatment of continuous veno-venous hemofiltration (CVVH)in refractoriness heart failure with renal insufficiency patients.Methods 35 hospitalized patients of refractori- ness heart failure with renal insufficiency were chosen from 2001 to 2006,25 patients were treated by CVVH,and other 10 patients were treated by regular hemodialysis.The serum BUN,Cr,left ventricle ejection fraction(LVEF),were observed before and after the therapy in each group.Results The clinical symptom were obviously improved in Group HF,and LVEF,cardiac output(CO) were both increased by continuous hemofiltration treatment(P<0.01).The clinical symptom were not obviously improved in control group,and LVEF,CO were hot increased.The complications by regular hemodialy- sis treatment were more than that of CVVH.Conclusion CVVH can keep hemodynamics stable during removing excess water and toxin and it is an effective and safe method to treat refractory heart failure with renal insufficiency.更多还原
