【摘要】 目的:探讨局部晚期的早期宫颈癌(IB2和IIA期)综合治疗的模式。方法:2000年1月至2002年10月,我们以新辅化疗后+放疗后,行宫颈癌根治术,治疗宫颈肿瘤≥4cm的局部晚期的早期宫颈癌224例,术后根据盆腔淋巴结转移、宫颈间质深层浸润、脉管内癌栓、组织分化差等高危因素给予化疗和外照射治疗。回顾性分析同期收治的IB2~IIA期宫颈癌100例,作为对照组,均行术前腔内后装放疗+宫颈癌根治术。结果:治疗组与对照组癌细胞消失率差异有显著性(P=0.006);癌细胞浅肌层浸润率差异也有显著性(P=0.002);深肌层浸润率差异无显著性(P=0.316)。盆腔淋巴结转移率比较:髂血管区>闭孔区>骶前区同时合并髂血管区。结论:新辅助化疗+术前后装联合手术治疗是局部晚期的早期宫颈癌综合治疗的较佳模式。宫颈癌术中有必要做骶前区淋巴结清扫。更多还原

【Abstract】 Objective:To explore combined modality treatment for locally advanced cervical carcinoma. Methods: from Jannuary 2000 to October 2002, we had taken radical hysterectomy of cervical cancer among 224 patients with locally cervical carcinoma after neoadjurant chemotherapy and radiotherapy. While 100 cases of stage ⅠB2 and Ⅱa as control which received radiotherapy before radical hysterectomy. Results: After operation pathological ,the rate of cancer cell disappear ance and shallow muscle layer invasion had significant difference between two groups (P<0.01),but it had no difference for deep muscle layer invasion (P>0.05) . Conclusion: Neoadjuvant chemotherapy plus. radiotherapy combined operation is a better treatment for locally advanced cervical carcinoma . It is necessary for radical hysterectomy of cervical carcinoma to cut off cacrum front lymphatic .更多还原
