健康成人与肝脏疾病患者血浆肉毒碱的检测

【摘要】 目的建立成人总肉毒碱(TC)和游离肉毒碱(FC)的参考范围;检测肝病患者血浆TC和FC水平,并探讨其临床意义。方法采用循环酶法测定35例慢性病毒性肝炎、30例急性病毒性肝炎和30例肝硬化患者血浆TC和FC水平,同时测定200名健康成年人血浆TC和FC水平作为对照。结果健康成人TC为(56.5±9.6)μmol/L、FC为(46.6±8.2)μmol/L,其中男性TC为(57.5±8.5)μmol/L,女性TC为(55.6±10.5)μmol/L,两者之间差异无统计学意义(P>0.05),FC男性为(48.4±7.2)μmol/L,水平略高于女性(44.7±8.9)μmol/L。慢性病毒性肝炎患者的TC为(46.9±14.8)μmol/L、FC为(36.3±9.8)μmol/L,急性病毒性肝炎患者的TC为(44.4±14.2)μmol/L、FC为(35.4±13.0)μmol/L,均低于健康成年人(P<0.01);肝硬化患者TC和FC水平与健康组差异无统计学意义(P>0.05)。结论TC水平男女之间差异无统计学意义,FC男性水平略高于女性;女性肉毒碱有随年龄增加的趋势。肝病患者可发生肉毒碱代谢异常。

【Abstract】 Objective To establish the reference range of total carnitine(TC) and free carnitine(FC) in Chinese healthy adults,and to explore its clinical significance in patients with liver diseases. Methods The levels of plasma TC and FC in 35 patients with chronic viral hepatitis,30 patients with acute viral hepatitis,30 patients with hepatocirrhosis,and 200 healthy controls were measured by enzymatic cycling method. Results The levels of plasma TC and FC in healthy adults were(56.5±9.6)μmol/L and(46.6±8.2)μmol/L respectively.The levels of plasma TC in male and female were(57.5±8.5)μmol/L and(55.6±10.5)μmol/L respectively,there was no significant difference(P>(0.05).) The levels of plasma FC in male and female were(48.4±7.2)μmol/L and(44.7±8.9)μmol/L.The levels of TC and FC decreased in chronic viral hepatitis(TC[46.9±14.8]μmol/L,FC[36.3±9.8]μmol/L) and acute viral hepatitis(TC[44.4±14.2]μmol/L,FC [35.4±13.0]μmol/L).The levels of plasma TC and FC in patients with hepatocirrhosis showed no difference compared with control group(P>0.05). Conclusions The levels of TC have no significant difference between male and female.Carnitine concentration in male is slightly higher than in female and carnitine level seems to increase with age in female.Abnormalities in carnitine metabolism are observed in various liver diseases and their cause is complex.
