【摘要】 目的:探讨血脂联素、抵抗素、瘦素及瘦素受体在多囊卵巢综合征(PCOS)抗高雄激素血症治疗和胰岛素增敏剂治疗中的变化。方法:27例PCOS患者采用达英-35(每片含醋酸环丙孕酮2mg、炔雌醇35μg)或达英-35联合二甲双胍治疗6个月,ELISA方法检测治疗前后的空腹血脂联素、抵抗素、瘦素及瘦素受体浓度,同时检测血糖和血清胰岛素、双氢睾酮(DHT)、脱氢表雄酮(DHEA)、硫酸脱氢表雄酮(DHEAs)、黄体生成激素(LH)和卵泡刺激素(FSH)等相关激素水平。结果:①所有患者的血DHT和DHEA在治疗后均明显降低(P<0.05);②单用达英-35组治疗后血抵抗素显著下降(P<0.05),血脂联素升高、瘦素受体降低,但治疗前后差异无显著性,而瘦素没有明显变化;③达英-35联合二甲双胍组治疗后,血脂联素、瘦素受体显著升高,瘦素显著下降(P<0.05),而抵抗素的下降差异无显著性。结论:脂肪细胞分泌的脂联素和瘦素在胰岛素增敏剂治疗后的明显改变,说明其可能参与PCOS胰岛素抵抗的发生发展,而抵抗素的作用尚待研究。更多还原

【Abstract】 Objective:To study the change of serum leptin and leptin receptor, adiponectin,and resistin levels in PCOS patients that treated by Diane-35 alone or Diane-35 combined Metformin.Methods:27 PCOS patients received Diane-35 alone or Diane-35 combined Metformin treatment for 6 months.The serum leptin and leptin receptor,adiponectin,and resistin levels were detected by ELISA before and after treatment,meantime,fasting plasma glucose,serum insulin and FSH,LH,T,DHT,DHEA,and DHEAs were examined.Results:The serum DHT and DHEA levels significantly decreased in all patients after treated by Diane-35 alone or with Metformin.In PCOS patients without IR,serum resistin levels significantly decreased after the Diane-35 treatment(P<0.05),but serum leptin,leptin receptor and adiponectin level had no significant change.In patients with IR,serum adiponectin and leptin receptor levels significantly increased and serum leptin levels decreased after treated by Diane-35 with Metformin(P<0.05).But serum resistin levels had no change.Conclusions:Adipocyte-derived factors leptin and adiponectin except resistin,as regulators of insulin metabolism,participates the pathogenesis of insulin resistance in PCOS.更多还原
