短期胰岛素泵强化治疗对新诊断2型糖尿病的影响

【摘要】 目的观察新诊断2型糖尿病短期使用胰岛素泵强化治疗后血脂、胰岛β细胞功能的变化。方法对新诊断2型糖尿病46例,用胰岛素泵进行持续皮下胰岛素输注(CSII)治疗2周,采用自身前后对照,分析评价治疗前后血糖、血脂、胰岛β细胞功能改变情况。结果46例患者经强化治疗2周,高血糖状态得以控制,空腹及餐后2h血糖均达良好控制,Homaβ较治疗前明显提高,HomaIR较治疗前明显下降。血清总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、甘油三脂(TG)均较治疗前明显降低,高密度脂蛋白胆固醇(HDL-C)有明显升高,差异有统计学意义。结论短期胰岛素泵强化治疗对新诊断2型糖尿病患者,具有快速良好控制血糖,改善胰岛β细胞功能和改善脂质代谢作用。

【Abstract】 Objective To observe the effect of short-term intensive insulin pump treatment on plasma glucose, lipid profiles and beta-cell function in newly diagnosed type 2 diabetic patients. Methods Forty-six newly diagnosed type 2 diabetic patients were treated with continuous subcutaneous insulin infusion (CSII) for two weeks. The levels of plasma glucose, lipid profiles and beta-cell function were compared before and after CSII. Results After intensive insulin pump treatment, excellent glycemic control was achieved; HOMA-B increased significantly, and HOMA-IR decreased significantly; the levels of total cholesterol (TC), low-density lipoprotein-cholesterol(LDL-C), triglycerides(TG) also decreased significantly, the level of high-density lipoprotein-cholesterol(HDL-C) was improved significantly. Conclusion The excellent glycemic control and improvement of beta-cell function and metabolism of lipid can be induced by short-term intensive insulin pump therapy in newly diagnosed type 2 diabetic patients.
