β-连环素表达及基因突变与子宫内膜癌发生发展相关性的研究

【摘要】 目的:研究β-连环素(β-cat)在子宫内膜癌组织中的表达及基因突变在子宫内膜癌发生中的作用。方法:采用免疫组化SABC法和SSCP-PCR法检测31例子宫内膜癌标本和10例正常子宫内膜中β-cat的表达及基因突变,并对实验结果进行统计学分析。结果:31例子宫内膜癌74%为质/核聚集表达,明显大于正常子宫内膜,P<0.05。其中,Ⅰ期为84%,明显高于Ⅱ、Ⅲ期(16%),P<0.05;内膜样腺癌为88%,明显高于非内膜样腺癌,P<0.01,且60%分化良好,P<0.05。不同肌层浸润深度间比较差异无统计学意义,P>0.05。SS-CP法检测出5例突变条带,均为FI-GOⅠ期的子宫内膜癌,且4例为内膜样腺癌,但差异无统计学意义,P>0.05。突变组均伴有β-cat聚集,与正常组相比差异有统计学意义,P<0.05。结论:β-cat聚集与子宫内膜癌发生有关,尤其与早期、分化良好的内膜样腺癌的发生有关,但与肌层浸润深度无关。基因突变是β-cat聚集的重要分子事件,是导致子宫内膜癌发生的重要通路之一。

【Abstract】 OBJECTIVE:To study the expression and genic mutation of β-catenin in endometrial carcinoma and its correlation with the development of endometrial carcinoma.METHODS:The immunohistochemical method and single-strand conformation polymorphism-polymerase chain reaction(SSCP-PCR)analysis were performed to detect the expression and genic mutation of β-catenin in 31 endometrial carcinoma and 10 nomal endometria specimens.RESULTS:The accumulation of β-catenin was detected in 74% of endometrial carcinoma and 30% of normal endometria.The statistical difference was significant,P<0.05.The cytoplasm or nuclear expression was observed more freguently in Stage Ⅰ(84%)than in Stage Ⅱ and Ⅲ(16%),P<0.05.In the endometrioid adenocarcinomas,the accumulation was observed more frequently than that in other histology types(P<0.01)and tend to occur in well-differenciation,P<0.05.By contrast,the accumulation pattern had no correlation with the level of myometrial infiltration,all P>0.05.The mutant bands of SSCP-PCR were detected in 5 samples,which were all endometrial carcinoma and all of them were Stage Ⅰ,despite the data had no statistical significance,P>0.05.Besides,all the mutant were accompanied by the accumulation and the difference was significant,P<0.05.CONCLUSIONS:The accumulation of β-catenin involves in the development of endometrial carcinomas,especially related to the endometrioid adenocarcinomas of early stage and well-differentiated type despite it has no connection with myometrial infiltration level.The genic mutation is a molecule event of accumulation of β-catenin.Genic mutation followed by accumulation of β-catenin is an important neuplasic pathway to endometrial carcinomas.
