术中自体血液回收对红细胞pH值和电解质的影响
Effects of Intraoperative Autotransfusion on pH Value and Electrolyte of Red Blood Cell
【摘要】 目的观察自体血液回收对红细胞pH值和电解质的影响。方法30例美国麻醉医师协会体格情况分级(ASA)为Ⅱ或Ⅲ级,估计出血量>400ml的手术患者,采用国产自体-2000型血液回收机行术中自体血液回收,取患者麻醉前(外周组)、血液回收机处理后(回收组)的红细胞各30份及库存10d的异体去白细胞悬浮浓缩红细胞(库存组)30份,分别检测其红细胞内外pH值及红细胞内K+、Na+的浓度。结果库存组红细胞内、外pH值均明显低于回收组和外周组(P<0.01),外周组红细胞内、外pH值明显低于回收组(P<0.01);3组红细胞内Na+、K+的浓度间差异无显著性意义(P>0.05)。结论术中自体血液回收可致红细胞内、外pH值明显升高;但对红细胞内K+、Na+的浓度无明显影响。
【Abstract】 Objective To observe the effects of intraoperative autotransfusion on pH value and electrolyte of red blood cell( RBC).Methods Thirty patients graded Ⅱ-Ⅲ by ASA( aged 20~60),who were estimated to have lost more than 400mL blood during surgical intervention ,were enrolled in this study.ZITI-2000 cell saver system was used for intraoperative blood salvage.We obtained thirty samples of blood before anesthesia(peripheral group) and thirty salvaged from field(salvaged group) and thirty stored concentrated RBCs (stored group).The pH value and the content of K+and Na+ of the RBC were detected.Results The pH value of the stored group was significantly lower than the salvaged group and the peripheral group,and the pH value of the peripheral group was significantly lower than that of the salvaged group(P<0.01).There was no significant difference in content of Na+ 、K+ among three groups(P>0.05).Conclusion The intraoperative autotransfusion can remarkably elevate the pH value of RBC,but there is no obvious effect on content of K+ and Na+.
