中轴线距离法在脊柱侧凸疗效评价中的应用

【摘要】 目的探讨中轴线距离法(简称中线法)在脊柱侧凸疗效评价中的应用价值。方法选择30例特发性脊柱侧凸患者的脊柱全长片(分别包括穿戴支具治疗前、支具穿戴在身时、以及去支具后的3次检查,共90帧),由3位医师采用Cobb法和中线法在PACS工作站上对所有X线片独立测量,每种测量方法对支具穿戴时和去支具后测量结果均与治疗前的测量结果相减,以确定有效率和矫正度。结果3位医师Cobb法测量时有效率和矫正度相互间比较差异均有显著性(P<0.05);而中线法测量的有效率及矫正率差异无显著性(P>0.05)。30例中,24例侧凸得到矫正,6例有加重,有效率80%;支具穿戴时的平均矫正率为(35.73±9.53)%,去支具后平均矫正率为(20.12±8.13)%。结论中线法具有测量点明确、重复性好、测量结果受主观因素影响小等优点,在脊柱侧凸的治疗随访中具有重要价值,值得试用。

【Abstract】 Objective To investigate the clinical significance of axis-line-distance technique(ALDT)in evaluating the curative effect of scoliosis.Methods The full spine imagings of 30 cases with idiopathic scoliosis were measured by 3 doctors with Cobb method and ALDT on PACS workstation,on three different occasions, such as brace prescription, initial in-brace, brace discontinuation. The availability and rectification was assessed by analyzing the difference between brace prescription and initial in-brace or brace discontinuation.Results There were significant measurement differences in 3 doctors with Cobb method measurement(P<0.05).With ALDT, there were no significant measurement differences for three observers in three different occasions(P>0.05),as well as the cure correction rate(P>0.05).80%(24/30) scoliosis were corrected. The average cure correction rate was(35.73±9.53)% when dressing the brace on body,and(20.12±8.13)% without the brace.Conclusions ALDT is worth implementing clinically in scoliosis because it is simple,reliable,and without significant measurement difference among the observers.
