13—18岁汉族正常(牙合)青少年上牙弓后段可利用间隙变化的纵向研究
A longitudinal study on changes of posterior available space of upper dental arch for Chinese adolescents with normal occlusion from 13 to 18 years of age
【摘要】 目的探讨13—18岁汉族正常骀青少年上牙弓后段可利用间隙的增龄性变化。方法对26名汉族正常骀青少年(男12人,女14人)13—18岁每年同期拍摄头颅侧位定位片,连续6年,进行 X 线头影测量纵向分析。结果(1)13—18岁上牙弓后段可用间隙的变化存在明显的性别差异,女性每侧间隙共增长3.29mm,男性共增长5.25mm;(2)13—18岁牙弓后段可利用间隙的增加与上颌骨的生长改建及上颌磨牙近中漂移有关;(3)女性比男性较早结束颌骨的生长改建,女性14岁、男性15岁上颌后段骨改建基本结束;13—18岁期间存在上颌磨牙近中漂移现象,第二、三磨牙的萌出力可能会加速其近中漂移。结论上牙弓可用间隙的增龄性变化具有年龄、性别等差异,临床上可参考本研究结果预测相关可用间隙。
【Abstract】 Objective The purpose of this study was to investigate the changes of available space of posterior dental arch in maxilla of the teenagers from 13 to 18 years of age,which would provide information for space analysis of whole dental arch in adolescents.Methods 26 adolescents (12males, 14females)with normal occlusion were selected.Sequential cephalograms of them were taken once a year, lasting for 6 years.Longitudinal cephalometric and statistic analysis were performed.Results (1) Significant differences of available space were found between males and females.The whole increase of available space from 13 to 18 years of age was 3.29mm in females and 5.25mm in males; (2)The increase of available space was made partly by growth of maxilla,and partly by mesial movement of the first molars. (3)The completion of maxillary remodeling in males lagged behind females about 1 year.No significant increase was found in girls beyond 14 years of age and in boys beyond 15 years of age.The mesial movement of dental arch existed all through the 6 years,which can probably be accelerated by the eruption of the second and third molars.Conclusion The prediction of available space should be based on age and sex.
