乳癌患者99mTc-MIBI显像及癌组织中P-糖蛋白、多药耐药相关蛋白测定
99mTc-MIBI imaging of breast cancer and its expression of p-glycoprotein and multidrug resistence-associated protein
【摘要】 目的:评价乳癌患者99mTc-甲氧基异丁基异腈(MIBI)显像结果,并检测癌组织中P-糖蛋白(P-gp)、多药耐药相关蛋白(MRP)的表达。方法:42例乳腺癌患者静脉注射99mTc-MIBI555~740MBq(15~20mCi),10min及3h后显像。计算早、晚期肿瘤部位平均放射性计数与对侧相应部位平均放射性计数比值(T/N)、清除率。术后标本采用免疫组织化学法检测P-gp、MRP的表达。结果:根据P-gp、MRP免疫组织化学结果将42例患者分为4组:A组P-gp阳性、MRP阳性,7例;B组P-gp阳性、MRP阴性,15例;C组P-gp阴性、MRP阳性,9例;D组P-gp阴性、MRP阴性,11例。早期T/N值,A组、B组与D组比较,差异均有统计学意义(P均<0.05),其余各组间差异无统计学意义(P均>0.05)。晚期T/N值,各组间差异无统计学意义(F=0.460,P=0.650)。清除率,A组与B组、C组、D组之间,B组、C组与D组之间,差异均有统计学意义(P均<0.05),而B组与C组之间,差异无统计学意义(P>0.05)。结论:乳癌99mTc-MIBI显像计算清除率可用于评估癌组织中P-gp、MRP的表达。
【Abstract】 Aim:To evaluate the relationship between the result of 99mTc-MIBI imaging of breast cancer and the expression of p-glycoprotein(P-gp) and multidrug resistence-associated protein(MRP).Methods:A total of 42 patients with breast cancer were given 555~740 MBq (15~20 mCi) 99mTc-MIBI intravenously. Breast imaging was performed 10 min(early phase) and 3 h(delayed phase) after the injection. The tumor-to-normal (T/N) ratio and wash-out rate (WR) were calculated on both early and delayed images.The expressions of P-gp and MRP in breast cancer tissues were examined by immunohistochemistry.Results:The 42 patients were divided into 4 groups according to the results of immunohistochemistry: group A, P-gp(+) and MRP(+), 7 patients;group B, P-gp(+) and MRP(-), 15 patients;group C, P-gp(-) and MRP(+), 9 patients;group D, P-gp(-) and MRP(-), 11 patients. The early T/N ratio was significant lower in group D than that in group A and B (P<0.05).The delayed T/N ratio had no significant difference among the 4 groups(F=0.460,P=0.650).There were significant differences on WR between group A and B, group A and C, group A and D, group B and D, group C and D (P<0.05).However, no significant difference was found between group B and C(P>0.05).Conclusion: WR calculated from 99mTc-MIBI imaging of breast cancer is useful for determining expression of P-gp and MRP.
