RNA病毒性脑膜炎的实验诊断研究
Research on Laboratory Diagnosis of RNA Virus Meningitis
【摘要】 目的:研究探索RNA病毒所致毒性脑膜炎(病脑)的临床实验诊断方法。方法:病毒性脑膜炎患者(病脑组)和非病毒性脑膜炎患者(对照组)为对象,应用反转录聚合酶链反应技术(RT-PCR)检测脑脊液标本中RNA病毒(包括肠道小RNA病毒组、柯萨奇病毒及风疹病毒),应用酶联免疫分析技术(ELISA)检测脑脊液和血液中柯萨奇病毒-IgM和风疹病毒-IgM。结果:RT-PCR方法的特异性为100%,阳性率分别为肠道小RNA病毒组35.3%、柯萨奇病毒29.8%、风疹病毒9.7%;而ELISA方法检测血清标本的特异性为93.7%(COX-IgM)和95.2%(RV-IgM),敏感性分别为38.2%和8.4%,检测脑脊液则无阳性发现。结论:RT-PCR是一种临床实验确证诊断病脑的有效而实用的方法,与ELISA方法(检测血清标本)联合应用,可以进一步提高实验诊断资料的质量。
【Abstract】 Objective: To probe clinical methods of laboratory diagnosis for the diagnosis of RNA virus meningitis.Method: The objects consisted of the patients with viral meningitis and the patients with non-viral meningitis.RNA virus(to include Picornavirous group,Coxsackie virus and Rubella virus) were detected by reverse transcriptase-polymerase chain reaction(RT-PCR) in cerebrospinal fluid.Coxsackie virus IgM and Rubella virus IgM were detected by ELISA in cerebrospinal fluid and blood.Result: The specificity of RT-PCR was 100% in cerebrospinal fluid;ELISA was 93.7% and 95.2% in blood.The positive rate of picornavirus group was 35.3%,Coxsackie virus 29.8% and Rubella virus 9.7%;Coxsackie virus IgM was 38.2% and Rubella virus IgM 9.7%.In cerebrospinal fluid,there was not positive resulte of IgM.Conclusion: RT-PCR is a useful method for the laboratory diagnosis of meningitis from RNA virus,to concert RT-PCR and ELISA make further improvement on the quality of laboratory material.
