【摘要】 目的探讨超声心动图在产前筛查胎儿先天性心脏病的临床价值。方法通过超声心动图对2126例孕妇进行产前筛查,观察胎儿四腔心切面、左右室流出道切面、主动脉弓、动脉导管弓切面及上下腔静脉长轴切面二维声像图,如发现胎儿心脏可疑病变,进一步做全套超声心动图检查。结果发现胎儿先天性心脏病25例,其中病例:心内膜垫缺损6例,三尖瓣闭锁并大动脉转位3例,大型室缺6例,法洛氏四联症2例,单心房2例,左心室发育不良3例,房间隔缺损2例,主动脉狭窄1例。通过引产后尸解及随访证实可疑先心病27例,漏诊2例,符合率为92.6%。结论应用常规超声心动图筛查胎儿先天性心脏病是可行的,在胎儿产前畸形筛查中具有重要临床应用价值,值得推广。更多还原

【Abstract】 Objective To research clinical value of fetal congenital heart defect screen with echocardiography. Methods The fetal two-dimension major axis section images of 2,126 pregnant woment were got by echocardiography in prenatal examination, including four-chamber view, outflow tract of left and right ventricle, aortic arch, arterial ductus arch, superior and inferior caval vein. Further in case suspicious congenital heart defect were found, all items of the echocardiography were detected. Results 25 cases of congenital heart defect were found out by echocardiography, including endocardial cushion defect(ECD)6 cases, tricuspid atresia(TA) and transposition of the great arteries(TGA)3 cases, ventricular septal defect (VSD) 6 cases, tetralogy of Fallot(TOF) 2 cases, Single atrium 2 cases, hypoplastic left ventricle(HLV) 3 cases, Atrial Septal Defect(ASD) 2 cases, coarctation of the aorta(COA) 1 case. 27 cases of suspicious congenital heart defect cases were made a definite diagnosis by autopsy after abortion in second trimester and follow-up; only 2 cases were missed diagnosis. The diagnostic accordance rate equal 92.6%. Conclusions The fetal congenital heart defect screen with routine echocardiography in prenatal examination is reasonable. It has important significance for clinical diagnosis and recommend valuable for clinical routine use.更多还原
