慢性鼻窦炎鼻咽部表面活性物质主要成分磷脂酰胆碱的变化

【摘要】 目的:探讨20例慢性鼻窦炎患者鼻咽部表面活性物质成分磷脂酰胆碱的变化,以反映对中耳和咽鼓管功能的影响。方法:采集本院耳鼻喉科慢性鼻窦炎患者和健康人鼻咽部灌洗液,用高效液相色谱方法对表面活性物质的成分磷脂酰胆碱的含量进行测定。结果:①两组鼻咽部均可测出磷脂酰胆碱。②慢性鼻窦炎组与对照组磷脂酰胆碱含量分别是(82.97±35.70)μg和(114.86±33.64)μg(P<0.05)。③慢性鼻窦炎各分型分期与对照组、各分型分期之间比较差异无统计学意义,但随分型分期进展,磷脂酰胆碱含量有降低的趋势。结论:①鼻咽部存在表面活性物质,可测出磷脂酰胆碱。②慢性鼻窦炎可引起鼻咽部磷脂酰胆碱含量减少,引起中耳、咽鼓管功能不全。

【Abstract】 Objective:To investigate the impacts of chronic sinusitis on phosphatidylcholine,a major biochemical component of nasopharyngeal surfactant,as to reflect how much the middle ear and eustachian tube function are affected.Methods: Nasopharyngeal lavage was performed in subjects with or without chronic sinusitis. Contents of phosphatidylcholine in the lavage liquid were measured using highperformance liquid chromatography.Results: ①For both groups,phosphatidylcholine was detectable in nasopharygeal surfactant.②Compared with controls,patients with chronic sinusitis had significantly lower level of phosphatidylcholine [(114.86±33.64)μg vs(82.97±35.70)μg,P<0.05].③The levels of phosphatidylcholine did not differ among types of chronic sinusitis; however,there was a trend towards reduced phosphatidylcholine along with more advanced stages of chronic sinusitis.Conclusion: ①Phosphatidylcholine is detectable in surfactant found in nasopharynx.②Chronic sinusitis may lead to lowered level of nasopharyngeal phosphatidylcholine,with the more advanced stages,the more reduction,thereby causing dysfunction of the middle ear and eustachian tube.
