斑点法测定血清转铁蛋白受体研究
A Study on Spot Assay for Transferrin Receptor in Serum Samples Dried on Filter Paper
【摘要】 目的:了解斑点法与常规方法测定血清转铁蛋白受体结果的相符程度,为血清转铁蛋白受体在机体铁营养状况评价的推广应用提供理论依据。方法:在贫血调查的基础上取60名12~16岁的中小学生静脉血,分离血清后分别对同一样本用常规方法和斑点法对血清转铁蛋白受体的浓度进行测定。同时随机抽取32份样品,定量滴涂在滤纸片上,在室温保存2周后采用斑点法测定血清转铁蛋白受体的浓度。结果:常规方法与斑点法测定血清转铁蛋白受体的浓度分别为26.8nmol/L和25.6nmol/L(P>0.05),两种方法测定结果呈直线相关(r=0.93,P<0.01)。以>26.5nmol/L作为铁缺乏的诊断标准,与常规法相比较,斑点法对铁缺乏诊断的敏感性和特异性分别为81.5%和100%。经斑点法处理的样品在室温(18~25℃)下保存2周后,其测定结果仍与常规方法保持了高度的一致性(Kappa=0.7277)。结论:斑点法与常规方法测定血清转铁蛋白受体浓度具有较高的一致性,由于斑点法在样品保存和运输上优于常规方法,因而在机体铁营养状况普查中具有重要意义和实际推广价值。
【Abstract】 Objective:To study the result consistency of filter spotted assay and routine method in determination of transferrin receptor for iron deficiency diagnosis,so as to provide theoretical basis for the popularization of serum transferring receptor(sTFR)as an index in the evaluation of body iron status.Method:Concentrations of sTFR from venous blood of 60 students aged 12-16 were tested with both filter spotted method and traditional method.In addition,32 serum samples were chosen randomly out of the 60 samples and put on the filter paper,kept at room temperatures.The sTFR concentrations of them were tested with filter paper method in two weeks.Results:The concentrations of sTFR tested with traditional method and filter spotted method were 26.13nmol/L and 25.87 nmol/L(P>0.05)respectively,which were linearly correlated(r=0.93,P<0.01).Using the concentration >26.5nmol/L as diagnosis standard for iron deficiency,and compared with traditional method,the sensitivity and specificity of filter spotted method were 81.5% and 100% respectively.The study results showed a consistency(Kappa=0.7277)between filter spotted method and traditional method,even after the samples were kept for 2 weeks at room temperatures(18-25℃).Conclusion:Filter spotted method is consistent with traditional method in determining the concentration of sTFR,and superior to the routine method in sample storage and transportation.Therefore,it is of vital importance in survey of body iron status and worth popularization after further studies.
