肝癌组织中CD146、微血管密度与临床病理关系的研究

【摘要】 目的:探讨肝细胞癌(hepatocellular carcinoma,HCC)组织中CD146(MCAM)和微血管密度(microvessel desity,MVD)与临床病理及血管生成的关系。方法:采用荧光实时定量聚合酶链反应(RealtimePCR)技术和免疫组化法检测60例HCC癌组织及癌旁肝组织中CD146mRNA及其蛋白的表达情况,用CD34标记免疫组化法检测MVD。结果:CD146mRNA在HCC组织中的表达水平(0.0406±0.0135)明显高于癌旁肝组织中的表达水平(0.0356±0.0127)(P<0.05);CD146蛋白在HCC组织中的阳性率为66.67%,而在癌旁组织中的阳性率为30%,两者差异有统计学意义(P<0.05)。癌组织的MVD为54.92±8.55/200倍视野,癌旁组织的MVD为21.36±6.63/200倍视野,两者差异有统计学意义(P<0.01)。CD146蛋白在HCC组织中的表达与临床分期、门静脉癌栓及肝外转移明显相关,MVD在HCC组织的水平与肝外转移、肿瘤直径、肿瘤个数相关。CD146表达阴性的HCC组织MVD(51.18±7.32/200倍视野)明显低于阳性组织MVD(56.28±8.88/200倍视野)(P<0.05)。结论:CD146高表达可能与HCC的发生、发展及术后复发有关;与HCC血管形成有关。

【Abstract】 Objective: To explore the relationship between CD146(MCAM), microvessel density of hepatocellular carcinoma and its clinic-pathology, and to explore their correlation with angiogenesis of HCC. Methods: Real time polymerase chain reaction (Real time PCR) and Immunohistochemisty was employed to determine the expression of CD146 and microvessel density (MVD) in 60 HCC tissues and their adjacent liver tissues. Results: The expression level of CD146 mRNA in the HCC tissue (0.040 6 0.013 5) was markedly higher than the adjacent liver tissue(0.0356±0.0127, P<0.01).Positive rates of CD146 in the HCC tissues were higher than those in the adjacent liver tissues (66.67% vs 30% , P<0.05). MVD in the HCC tissue was significantly higher than in the adjacent liver tissue (54.92±8.55 vs. 21.36±6.63,P<0.05). The expression of CD146 in the HCC tissue was significantly correlated with the clinical stage, the Portal Vein Tumor Thrombus and the presence of extrahepatic metastasis. The expression of MVD in the HCC tissue was significantly correlated with the presence of extrahepatic metastasis, the diameter of tumor and the number of tumor. MVD in negative expression of CD146 in the HCC tissues were lower than those in the positive HCC tissues (51.18 7.32 vs 56.28 8.88, P<0.05). Conclusion: The high expression of CD146 in HCC tissues may be related to the initiation and progression and the recurrence of HCC, CD146 may play important roles in angiogenesis of HCC.
