急性心肌梗死心电图对梗塞相关动脉的预测价值

【摘要】 目的:探讨急性心肌梗死患者体表心电图对梗塞相关动脉预测的价值。方法:对120例急性心肌梗死患者的心电图与冠脉造影结果进行比较。结果:前间壁、前壁、广泛前壁心肌梗死其左前降支病变阳性率96.6%;下壁心肌梗死其右冠脉病变阳性率83%,左回旋支病变阳性率60%;下后壁心肌梗死其右冠脉病变阳性率89%,左回旋支病变阳性率67%;右室梗死其右冠脉病变阳性率100%;下壁合并右室梗死其右冠脉病变阳性率88%;正后壁梗死3例均显示为三支病变。结论:心肌梗死心电图定位与其相关冠脉病变关系密切,虽然有一定的局限性,仍不失为一项重要的评估方法。

【Abstract】 Objective:To explore the predictive value of surface electrocardiogram (ECG) for infarct-related artery in pa- tients with acute myocardial infarction.Methods:120 patients with acute myocardial infarction were observed;the results of ECG and coronary angiography were compared.Results:The positive rate of left anterior descending lesion was 96.6% in ante- ro-septal wall,anterior wall and extensive anterior wall myocardial infarction;the positive rate of right coronary artery and left circumflex artery lesions were 83% and 60% in inferior wall myocardial infarction;the positive rate of right coronary artery and left circumflex artery lesions were 89% and 67% in descending posterior wall myocardial infarction;the positive rate of right cor- onary artery lesion was 100% in right ventricular myocardial infarction;the positive rate of right coronary artery lesion was 88% in inferior wall combined with right ventricular myocardial infarction;the positive rate of coronary artery triple vessel lesion was 100% in positive posterior wall myocardial infarction.Conclusion:Coronary artery lesion is closely related with the ECG of myo- cardial infarction.Though this method has certain limitations,but it is an important evaluating method for coronary artery lesion.
