开颅手术前微创穿刺减压救治脑疝期外伤性颅内血肿42例分析
Analysis of 42 cases with traumatic intracranial hematomas at the stage of brain hernia treated with minimally invasive decompression by puncture before craniotomy
【摘要】 目的探讨脑疝期颅内血肿术前快速微创穿刺抽吸减压术的临床效果。方法对脑疝期颅内血肿42例采用术前微创穿刺抽吸血肿减压,然后再进行常规骨瓣开颅血肿清除术。结果术前微创法吸出血肿10~80mL,从确定手术至微创抽出积血减压所需时间约5~15min,平均为10min。结论对脑疝期颅内血肿患者,术前微创穿刺减压能迅速缓解颅内高压,降低死亡率及致残率。
【Abstract】 Objective To study the clinical effects of rapid preoperative minimally invasive decompression by puncture and aspiration for intracranial hematomas at the stage of brain hernia. Methods The 42 cases were treated with preoperative minimally invasive decompression by puncture and aspiration for intracranial hematomas at the stage of brain hernia and then conventional bone flap craniotomy was performed on them for evacuation of hematomas.Results A total of 10-80 mL hematomas was aspirated by preoperative minimally invasive decompression and the duration from the time that the decision of performing the operation was made to effective decompression by minimally invasive aspiration of hematomas was 5-15 min,10 min on average.Conclusion Preoperative decompression by puncture and aspiration shall rapidly alleviate intracranial hypertension,and thus, reduce mortality and disability rates in patients with intracranial hematomas at the stage of brain hernia.
