应用SELDI技术检测弥漫大B细胞淋巴瘤预后相关的特异蛋白质谱

【摘要】 目的应用蛋白芯片表面加强激光解吸电离-飞行时间-质谱(SELDI-TOF-MS)术检测弥漫大 B 细胞淋巴瘤(DLBCL)患者预后相关蛋白标志。方法 SELDI 技术 wcx2芯片检测62例 DLBCL患者蛋白质谱,生物化学方法测定乳酸脱氢酶(LDH),酶联免疫吸附法(ELISA)测定β2-微球蛋白(β2-MG)和 CA125。分析健康人、患者疾病缓解期与患者病情恶化、复发时的差异蛋白质谱,进行生存期分析,同时与国际预后指标(IPI)、LDH、CA125和β2-MG 作相关性分析。结果 20例患者在病情恶化、复发和死亡前出现11.000×103~12.000×103蛋白质谱峰(LGT)高表达,阳性组生存期明显低于阴性组的生存期(P=0.0002)。IPI 低中危组中 LGT 蛋白阳性者生存期比阴性者生存期明显短(P=0.0034)。伴随的其他异常蛋白质谱:6.107×103、7.974×103及8.067×103与 LGT 蛋白呈正相关。LDH、CA125、β2-MG 三者在 LGT 蛋白质谱表达组比阴性组显著增高(P<0.05),而且 LGT 与 LDH、CA125及β2-MG 呈正相关(P<0.01)。结论 LGT 蛋白与6.107×103、7.974×103和8.067×103三组蛋白质联合检测,有望作为 DLBCL 预后不良的特异性血清学蛋白标志,可能补充 IPI 的不足。

【Abstract】 Objective To screen the specific serum biomarker in prognostic factors of diffused large B cell lymphoma.Methods We screened 92 serums sample including 62 diffused large B cell lymphoma (DLBCL)patients and 30 normal control subjects by surface-enhanced laser desorption/ionization-time of flight mass spectrometry(SELDI-TOF-MS)technique and weak cation exehanger(WCX2)protein chip,and de- tected their LDH-level by biochemical measurement,β2-MG and CA125 level by ELISA.The significant dif- ferent protein spectrometry were analyzed between normal control and the DLBCL patients who were in remis- sion stage,replased and worse condition.The time survival and the log-rank test were used to statistical anal- ysis.The correlation analysis with protein spectrometry,IPI(international prognosis index),LDH,β2-MG and CA125 were analyzed with spearman.Results The protein profiles revealed that a prominent peaks with M/Z 11×103 12×103(LGT)were significantly increased in 20 DLBCL patients who were in replased,worse condition or before they died.These patients" survival time were significantly shorter than whom LGT peaks were not in- creased(P<0.01).The patients survival time in IPI low or middle group whose LGT peak were positive were shorter than whom LGT were negative(P<0.01).The LGT eompanied peaks in 6.107×103,7.974×103 and 8.067×103 were increased,and these peaks were positive correlation with LGT(P<0.01).LDH,β2-MG and CA125 in LGT positive group were higher than that in negative group(P<0.05),and LDH,β2-MG,CA125 were positive correlation with LGT(P<0.01).Conclusion Our findings suggested that application of SELDI- TOF-MS can be of great potential for DLBCL prognosis screening.LGT protein and 6.107×103,7.974×103, 8.067×103 protein increased can be the specific worse prognosis biomakers for DLBCL patients,and these four makers would be complementary to IPI.
