【摘要】 目的探讨survivin、p53和bcl-2蛋白在非霍奇金淋巴瘤(NHL)中的表达与肿瘤细胞凋亡和预后的关系。方法应用TdT介导的dUTP缺口末端标记(TUNEL)技术和免疫组织化学SP法,检测82例NHL和17例良性淋巴结病变中细胞凋亡和survivin、p53和bcl-2蛋白的表达水平。结果40.2%(33/82)NHL表达survivin,在BCL、TCL的表达阳性率分别为46.5%、33.3%,而仅在11.8% (2/17)良性淋巴结病变中弱阳性表达,二者差异有统计学意义(P<0.05),但与NHL的免疫表型无关(P>0.1)。NHL中survivin的表达与突变型p53蛋白积聚(P<0.05)和肿瘤细胞凋亡的下降相关(P<0.001),但与bcl-2蛋白表达无关(P>0.1),且阳性表达患者的平均生存时间明显短于无表达患者(P<0.05)。结论survivin基因可能通过其凋亡抑制功能在NHL的发生、发展中有一定作用,且其与突变型p53蛋白的积聚显著相关,但与bcl-2蛋白表达无关;并可能是NHL的一个新的预后不良因子。更多还原

【Abstract】 Objective To explore the expression of survivin,p53,bcl-2 gene and its relation with tu- mor cell apoptosis and prognosis of non-Hodgkin lymphomas.Methods Tdt-mediated UTP nick end label- ing (TUNEL) and immunohistochemistry were used to study cell apoptosis and expression of survivin,bcl-2, p53 genes in 82 non-Hodgkin lymphoma samples and 17 benign lymph node pathological changes.Results Survivin was positive in 41 of 106(38.7%) cases of non-Hodgkin lymphomas(46.5%,33.3% in bcl and tcl respectively),but its expression was detected only in 2 of 17 cases of benign lymph node lesions (P<0.05). The expression of survivin was irrelevant to the immune phenotype of non-Hodgkin lymphoma (P>0.1),how- ever,highly correlated with accumulation of mutant p53 protein and reduction of tumor cell apoptosis (P<0.05),but not bcl-2 expression.Furthermore,the mean survival period was significantly shorter in patients ex- pressing survivin as compared with the survivin-negative group.Conclusion Apoptosis inhibition by overex- pression of survivin might be involved in the onset and process of non-Hodgkin lymphoma and its expression related to overexpression of p53,but not bcl-2 protein.The expression of survivin might be a new unfavorable prognostic factor of non-Hodgkin lymphoma.更多还原
