【摘要】 目的　为了解我院围产儿先天性心脏病发病情况。方法　对 2 0 0 0年 1月～ 2 0 0 3年 12月大连市妇幼保健院分娩的围产儿 35 0 6 8例进行前瞻性调查。结果　查出先心病 4 87例 ,其中彩色多普勒超声心动图诊断 4 6 7例 ,尸检确诊 2 0例。发病率 13.9‰。以单纯性室间隔缺损 (VSD) 2 0 .95 %和房间隔缺损 (ASD) 18.0 7%居多 ,其中又以 .膜部VSD和Ⅱ孔型ASD最多。复合型畸形占 4 1.6 8% ,以ASD +VSD(12 .73% )居多。出生～ 7d发生心力衰竭 5 9例。围产儿死亡 5 2例 ,死亡率1.4 8‰ ,占 1岁以内先心病死亡总数的 6 7.5 3%。以青紫型复杂畸形为主。结论　先心病的发生与母亲早期呼吸道病毒感染有关 (χ2 =2 0 .98,P <0 .0 1,相对危险度 5 .99)。PDA的发生与早产儿密切相关 (χ2 =2 9.1,P <0 .0 1,相对危险度 5 .0 1)。更多还原

【Abstract】 Objective: To understand the occurrence of congenital heart disease (CHD) in our hospital. Methods: In a 4-year-prospective study the authors investigated 35068 perinatal infants. Results: In which 487 were diagnosed as having CHD by color Doppler blood flow mapping (CDFM) and autopsy. The incidence of CHD was 13.9‰.One hundred and two case ( 20.95%) had simple ventricular septal defect (VSD), the incidence of which ranked the first. Eighty-eight case(18.07%) had simple atrial septal defect (ASD), which ranked the second of incidence. Among them complex deformity was 41.68%. Fifty-nine case deveopled cardiac failure during the fist week, and 52 died within perinatal period. The mortality was 1.48‰.Most of those who died had complex deformity with phyoxemia. Conclusion: Survey of 487 case of CHD showed that respiratory infection during early pregnancy was correlated greatly with the incidence of CHD. Premature infants was correlated closely with the incidence of patent ductus arteriosus.更多还原
