【摘要】 目的　探讨18F 氟代脱氧葡萄糖(18F FDG)符合探测显像在头颈部肿物诊断中的临床应用价值。方法　回顾性分析38例经病理及临床随访证实头颈部肿物患者的符合线路显像结果,观察各种病变对于18F FDG的摄取情况,并计算其T/N值。结果　38例患者,恶性淋巴瘤13例, 10例阳性;颈部淋巴结转移癌8例,全部显像阳性;10例原发肿瘤中1 例未显影;7例良性病变患者中6例呈阳性,即真阳性27 例,真阴性4 例,假阳性6 例,假阴性1 例。敏感性96.4%(27/28),特异性40.0%(4/10),准确性81.6%(31/38)。结论　18F FDG 符合显像诊断头颈部肿瘤具有较高的敏感性和准确性,多床位显像有助于肿瘤的分期。紧密结合临床,认真分析病灶的显像特征,有利于提高诊断的特异性。更多还原

【Abstract】 Objective To investigate the clinical value of 18 F-FDG imaging with coincidence SPECT in detecting head and neck lesions. Methods Analyze 38 cases’ images of 18 F-FDG imaging with coincidence SPECT. All 38 cases with head and neck lesions had been confirmed by pathology or by follow-up interviews. Observe the uptake of the lesions and calculated the T/N. Results Ten of 13 patients with malignant lymphoma had positive images. All images of 8 patients with metastases in neck showed positive.One of 10 patients with primary tumors showed negative images. Seven patients had benign lesions in head and neck and 6 of them had positive images. The sensitivity, specificity and accuracy were 96.4%, 40.0% and 81.6%, respectively. Conclusion 18 F-FDG imaging with coincidence SPECT is of high sensitivity and accuracy in detection of head and neck lesions, and whole body imaging is also helpful in staging of tumor. Integrated consideration of the clinical information and image characters are important to improve specificity.更多还原
