自然组织谐波和彩色多普勒超声对成人肠套叠的诊断价值

【摘要】 目的 探讨自然组织谐波结合彩色多普勒超声诊断成人肠套叠的实用价值。方法 使用 3. 5MHz和 8. 0MHz探头对 22例因腹部疼痛或包块的患者,做二维基波、自然组织谐波和彩色多普勒超声检查,分析肠套叠的声像图特征,套叠类型及病因。结果 22例经手术和病理证实。全部资料超声图像典型,超声定位符合率为 95. 5% (21 /22),超声对病因的发现率为 90. 9% (20 /22)。其中小肠 小肠型 7例,回肠 结肠型 8例,结肠 结肠型 7例。手术病理证实为肠道脂肪瘤 4例,小肠间质瘤 1例,肠道血管瘤 2例,炎性纤维样息肉 2例,Peutz Jeghers型错构瘤性息肉 2例,肠道淋巴瘤 2例,肠癌 6例,盲肠炎症 1例,血友病肠壁血肿 1例,Meckel’s憩室 1例。结论 自然组织谐波较基波能更清晰地显示病变,彩色多普勒超声对判断套叠肠管的缺血有重要帮助,超声在发现肿瘤等病因上有较高敏感性。

【Abstract】 Objective To investigate the value of native tissue harmonic imaging and color Doppler imaging (CDI) in diagnosing adult intussusception. Methods Fundamental imaging, native tissue harmonic imaging and CDI were performed in 22 patients with abdominal pain or abdomen mass. The probe frequencies were 3.5 MHz and 8.0 MHz. Ultrasonographic characteristics, types and causes of adult intussusception were analyzed. Results All the diagnosis were confirmed by operation and pathology.The ultrasonographic features of all the cases were typical. The location of the intussusception could be shown correctly in 21 of 22 patients (95.5%). The causes of intussusception were diagnosed in 20 of 22 patients (90.9%). There were 7 cases with small bowel intussusception, 8 with ileocolic intussusception, and 7 with large bowel intussusception. Causes of intussusception were as following: 4 cases with intestine lipoma, 1 with stromal tumor, 2 with intestinal hemangioma, 2 with inflammatory fibroid polyp, 2 with hamartomatous polyps of the Peutz Jeghers type, 2 with intestinal malignant lymphoma, 6 with intestinal carcinoma, 1 with cecum inflammation,1 with hemophilia, and 1 with Meckel’s diverticulum. Conclusion Native tissue harmonic imaging shows lesions more clearly than fundamental imaging. CDI plays important role in estimating intestine ischemia. US is sensitive in detecting neoplasm of adult intussusception.
