铜绿假单胞菌产酶与耐药相关性的初步研究
Research of relationship between enzyme-producing and drug resistance of pseudamonas aeruginosa
【摘要】 目的探讨铜绿假单胞菌产AmpC酶、金属酶与其耐药的相关性。方法从湘雅医院各临床科室送检的标本中分离出82株铜绿假单胞菌,采用纸片扩散法检测AmpC酶,纸片协同法检测金属酶,并选用7种广谱抗生素进行药敏实验。结果82株铜绿假单胞菌中,AmpC酶和金属酶阳性率分别为32.9%和8.5%。AmpC酶阳性菌株对头孢他啶(CAZ),头孢他啶/棒酸(CDO2),头孢塞肟(CTX),头孢塞肟/棒酸(CDO3),氧氟沙星(OFX),阿米卡星(AK)表现高度耐药,对亚安培南(IPM),头孢比肟(FEP),氨曲南(ATM)敏感;金属酶阳性菌株仅对阿米卡星(AK)敏感,对其余抗生素表现高度耐药。结论湘雅医院临床标本分离的铜绿假单胞菌产AmpC酶和金属酶阳性率高,两种酶与铜绿假单胞菌多重耐药性相关。铜绿假单胞菌产酶的检测有助于控制院内感染和指导临床用药。
【Abstract】 s:[Objective] In order to control the hospital infection and give help to clinical therapy,we investigate the relationship between drug resistance and AmpC enzyme,metallo-β-lactamase produced by Pseudamonas Aeruginosa.[Methods] The 82 Pseudamonas Aeruginosa strains were separated from clinical samples of Xiangya hospital.The AmpC enzyme was determined by a Kirby-Bauer agar disk diffusion method and the metallo-β-lactamase was determined by agar disk cooperation method,the drug test used by seven antibiotics was established to detect the relationship between enzymes and drug resistance.[Result] Of the 82 Pseudamonas Aeruginosa strains,the positive rates of AmpC enzyme and metallo-β-lactamase were 32.9% and 8.5% respectively.AmpC enzyme-positive strains appeared resistant to CAZ,CDO2,CTX,CDO3,OFX,AK and showed susceptible to FEP,IPM,ATM.Metallo-β-lactamase-positive strains only showed susceptible to AK,but showed resistant to other antibiotics.[Conclusion] The screen rates of enzyme-producing strains are high in our hospital clinical isolates of Pseudamonas Aeruginosa.The two enzymes are correlated to the drug resistance of Pseudamonas Aeruginosa.The detections of the two enzymes give help to control the hospital infection and clinical therapy.
